无头双加压螺钉内固定在踝关节骨折手术治疗中的应用

摘　　要：目的:比较无头双加压螺钉内固定和普通空心钉内固定在踝关节骨折治疗中的应用效果。方法:对本院2018年1月~2022年12月进行手术治疗的72例踝关节骨折患者进行回顾性分析。其中采用无头双加压螺钉内固定的40例纳入观察组,另外32例采用普通空心钉内固定纳入对照组。比较两组治疗前后踝关节活动度、手术相关指标、手术并发症发生情况以及治疗后的踝-后足功能。结果:两组治疗后3个月踝-后足功能优良率分别为92.5%和71.9%,观察组更高(P<0.05)。观察组术后6个月踝关节旋前、旋后、屈、伸活动度均高于治疗前和对照组治疗后6个月的活动度(P<0.05)。观察组手术时长、骨折愈合时长和完全负重时长均较对照组更短(P<0.05),观察组术中出血量少于对照组(P<0.05)。两组手术并发症总发生率分别为5.0%和25.0%,观察组更低(P<0.05)。结论:无头双加压螺钉内固定较普通空心钉内固定创伤更小,手术并发症更少,术后骨折愈合更快,踝-后足功能恢复更好。Objective:To compare the effect of headless double pressure screw internal fixation and common hollow nail internal fixation in the treatment of ankle fracture.Methods:A retrospective analysis was performed on 72 patients with ankle fracture treated by concurrent operation in our hospital from January 2018 to December 2022.Among them,40 cases of internal fixation with headless double pressure screws were included in the observation group,and 32 cases of internal fixation with ordinary hollow nails were included in the control group.Compare the range of motion of the ankle joint,surgical related indicators,incidence of surgical complications,and ankle hind foot function after treatment between two groups before and after treatment.Results:After 3 months of treatment,the excellent and good rates of ankle hind foot function in both groups were 92.5%and 71.9%,respectively,with higher rates in the observation group(P<0.05).The range of motion of ankle joint pronation,supination,flexion,and extension in the observation group at 6 months after surgery was higher than that before treatment and in the control group at 6 months after treatment(P<0.05).The observation group had shorter surgical duration,fracture healing duration,and complete weight-bearing duration compared to the control group(P<0.05),and the observation group had less intraoperative bleeding than the control group(P<0.05).The total incidence of surgical complications in the two groups was 5.0%and 25.0%,respectively,with the observation group having a lower incidence(P<0.05).Conclusion:Compared with conventional hollow nail internal fixation,headless double pressure screw fixation has less trauma,fewer surgical complications,faster fracture healing and better ankle-posterior foot functional recovery.
