摘 要:目的:评估高黏度骨水泥填充结合经皮椎体成形术(PKP)对老年胸腰段椎体压缩性骨折患者的骨水泥渗漏率及椎体恢复效果的影响。方法:本研究回顾性分析2020年1月~2023年12月在本院收治的80例老年胸腰段椎体压缩性骨折患者,根据治疗方案的不同,将患者分为两组:实验组(40例,意大利高黏度骨水泥填充+PKP)与对照组(40例,常规骨水泥填充+PKP)。收集并比较两组患者的临床数据,包括术后24h内骨水泥渗漏率、术后疼痛评分(Visual Analog Scale,VAS)、椎体高度恢复率及术后随访的功能评估(Oswestry功能障碍指数)。结果:实验组术后24h内骨水泥渗漏率为10.00%,显著低于对照组的30.00%,差异具有统计学意义(P<0.05)。椎体高度恢复率方面,实验组85.00%,而对照组为65.00%,差异显著(P<0.05);术后疼痛评分方面,实验组术后第1?天和第7?天的VAS评分显著低于对照组(P<0.05);术后随访3个月及6个月时,实验组的Oswestry功能障碍指数改善程度显著低于对照组(P<0.05)。结论:高黏度骨水泥填充结合PKP在老年胸腰段椎体压缩性骨折的治疗中,能够有效降低骨水泥渗漏的发生率,并改善患者的术后疼痛及椎体恢复情况。Objective:To evaluate the effect of high viscosity bone cement filling combined with percutaneous vertebroplasty(PKP)on bone cement leakage rate and vertebral recovery in elderly patients with thoracolumbar vertebral compression fractures.Methods:This study retrospectively analyzed 80 elderly patients with thoracolumbar vertebral compression fractures admitted to our hospital from January 2020 to December 2023.According to different treatment plans,the patients were divided into two groups:the experimental group(40 cases,high viscosity bone cement filling+PKP)and the control group(40 cases,conventional bone cement filling+PKP).Collect and compare clinical data of two groups of patients,including bone cement leakage rate within 24 hours after surgery,postoperative pain score(Visual Analog Scale,VAS),vertebral height recovery rate,and functional evaluation of postoperative follow-up(Oswestry Disability Index).Results:The bone cement leakage rate within 24 hours after surgery in the experimental group was 10.00%,significantly lower than the control group’s 30.00%,and the difference was statistically significant(P<0.05).In terms of vertebral height recovery rate,the experimental group had 85.00%,while the control group had 65.00%,with a significant difference(P<0.05);In terms of postoperative pain scores,the VAS scores of the experimental group on the first and seventh day after surgery were significantly lower than those of the control group(P<0.05);At 3 and 6 months of postoperative follow-up,the improvement of Oswestry functional impairment index in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:The combination of high viscosity bone cement filling and PKP can effectively reduce the incidence of bone cement leakage and improve postoperative pain and vertebral recovery in the treatment of elderly thoracolumbar vertebral compression fractures.
