颈部血管超声在脑梗塞合并颈动脉狭窄诊断中的应用

摘  要:目的:探究颈部血管超声在脑梗塞合并颈内、外动脉狭窄诊断中的应用。方法:选择2022年1月~2024年1月本院收治的100例疑似脑梗塞合并颈内、外动脉狭窄患者作为对象进行研究,所有患者接受颈部血管超声和经颅多普勒超声检查,以脑部血管造影为金标准,对比不同检查方式对脑梗塞合并颈内、外动脉狭窄的诊断效能及相关影像学指标。结果:经金标准检查,100例患者中,74例确诊为脑梗塞合并颈内、外动脉狭窄,经颅多普勒超声检查确诊的患者有63例,颈部血管超声检查确诊的患者有72例;颈部血管超声检查的灵敏度、准确率、阳性预测值、阴性预测值均高于经颅多普勒超声检查(P<0.05);颈部血管超声检查颈内、外动脉狭窄程度高于经颅多普勒超声检查(P<0.05)。结论:颈部血管超声在脑梗塞合并颈内、外动脉狭窄的诊断中具有较高的灵敏度和准确率,能更准确地评估颈内、外动脉狭窄程度,可作为脑梗塞合并颈内、外动脉狭窄诊断的首选影像学方法,为临床提供更为准确、可靠的诊断依据。Objective:To investigate the utilization of cervical vascular ultrasonography in diagnosing cerebral infarction in conjunction with internal and external carotid artery stenosis.Methods:A cohort of 100 patients,suspected of having cerebral infarction accompanied by internal and external carotid artery stenosis,admitted to our hospital from January 2022 to January 2024,were enrolled in this study.All patients underwent cervical vascular ultrasonography and transcranial Doppler ultrasonography.The diagnostic performance and pertinent imaging parameters of these modalities in detecting cerebral infarction with internal and external carotid artery stenosis were compared,using cerebral angiography as the gold standard.Results:Based on the gold standard examination,74 out of the 100 patients were confirmed with cerebral infarction in conjunction with internal and external carotid artery stenosis.Among them,63 cases were diagnosed via color Doppler ultrasonography,while 72 cases were diagnosed via cervical vascular ultrasonography.Cervical vascular ultrasonography demonstrated higher sensitivity,accuracy,positive predictive value,and negative predictive value compared to color Doppler ultrasonography(P<0.05).Cervical vascular ultrasonography also revealed greater common carotid artery intima-media thickness and a higher degree of internal and external carotid artery stenosis compared to color Doppler ultrasonography(P<0.05).Conclusion:Cervical vascular ultrasonography exhibits high sensitivity and accuracy in diagnosing cerebral infarction associated with internal and external carotid artery stenosis.It can more precisely assess the intima-media thickness of the common internal and external carotid artery and the severity of internal and external carotid artery stenosis.Therefore,it can be considered as the preferred imaging modality for diagnosing cerebral infarction with internal and external carotid artery stenosis,providing a more accurate and reliable diagnostic foundation for clinical practice.
