摘　　要：目的:探讨CT灌注成像技术(CTP)在急性缺血性脑卒中(AIS)诊断中的临床价值。方法:选取2022年1月~2024年1月本院收治的82例疑似AIS患者,均行CTP、数字减影血管造影(DSA)检查,并以DSA为金标准,统计AIS检出情况及CTP的诊断效能,并用Kappa检验验证CTP诊断AIS与金标准的一致性;比较AIS患者患侧与健侧脑血流量(CBF)、平均通过时间(MTT)、达峰时间(TTP)及脑血容量(CBV)差异。结果:82例疑似患者最终DSA确诊60例,其中CTP检出59例,检出率为98.33%(59/60);CTP诊断AIS的灵敏度为96.67%(58/60)、特异度为95.45%(21/22)、准确度为96.34%(79/82)、阳性预测值为98.31%(58/59)、阴性预测值为91.30%(21/23);Kappa检验显示,CTP诊断与金标准的Kappa值=0.908,一致性极好。结论:CTP在AIS诊断中价值高,便于早期迅速识别AIS,实现针对性治疗,改善患者预后。Objective:To explore the clinical value of CT perfusion imaging(CTP)in the diagnosis of acute ischemic stroke(AIS).Methods:82 suspected AIS patients admitted to our hospital from January 2022 to January 2024 were examined by CTP and digital subtraction angiography(DSA),and the detection of AIS and the diagnostic efficiency of CTP were counted with DSA as the gold standard.Kappa test was used to verify the consistency between CTP diagnosis of AIS and the gold standard.The differences of cerebral blood flow(CBF),mean transit time(MTT),time to peak(TTP)and cerebral blood volume(CBV)between the affected side and the healthy side were compared.Results:Of the 82 suspected patients,60 cases were finally diagnosed by DSA,among which 59 cases were detected by CTP,the detection rate was 98.33%(59/60).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CTP in diagnosing AIS were 96.67%(58/60),95.45%(21/22),96.34%(79/82),98.31%(58/59)and 91.30%(21/23).Kappa test showed that the Kappa value of CTP diagnosis and gold standard was 0.908,and the consistency was excellent.Conclusion:CTP is of high value in the diagnosis of AIS,which is convenient for early and rapid identification of AIS,targeted treatment and improved prognosis of patients.
