单孔腹腔镜疝囊高位结扎术治疗小儿腹股沟疝的临床效果及对睾丸发育的影响

摘　　要：目的:研讨单孔腹腔镜疝囊高位结扎术(LH)治疗小儿腹股沟疝(IH)的临床效果及对睾丸发育的影响。方法:选取2021年6月~2023年3月经本院确诊为IH的小儿患者86例,按照随机数表法分为两组,每组43例。对照组予以传统疝囊高位结扎术(OH)处理,观察组予以单孔LH术治疗,比较两组围术期基础指标、睾丸发育情况、机体应激指标[去甲肾上腺素(NE)、丙二醛(MDA)与皮质醇(Cor)]及手术安全性。结果:观察组各围术期基础指标(手术时间、切口大小、术中出血量及平均住院日)均优于对照组(P<0.05)。术后,观察组患侧睾丸体积大于对照组(P<0.05)。术后,观察组3项机体应激指标(NE、MDA及Cor)检测值均低于对照组(P<0.05)。观察组术后出现并发症(阴囊血肿、切口缝线反应等)的患儿占比(6.98%)少于对照组(23.26%),差异有统计学意义(P<0.05)。结论:单孔LH手术用于IH患儿的治疗近期效果较好,并发症较少,且能够有效减轻机体应激反应,促进睾丸发育。Objective:Discuss the clinical efficacy of single port laparoscopic high ligation of hernia sac(LH)in the treatment of pediatric inguinal hernia(IH)and its impact on testicular development.Methods:86 pediatric patients diagnosed with IH in our hospital between June 2021 and March 2023 were selected and included in two groups using a random number table method,with 43 cases in each group.The control group received traditional high ligation of hernia sac(OH)treatment,while the observation group received single port LH surgery treatment.The perioperative basic indicators,testicular development,body stress indicators[norepinephrine(NE),malondialdehyde(MDA),and cortisol(Cor)]and surgical safety were compared between the two groups.Results:The basic perioperative indicators(surgical time,incision size,intraoperative bleeding,and average length of hospital stay)of the observation group were better than those of the control group(P<0.05).After surgery,the volume of the affected testicles in the observation group was larger than that in the control group(P<0.05).After surgery,the detection values of three stress indicators(NE,MDA,and Cor)in the observation group were lower than those in the control group(P<0.05).The proportion of postoperative complications(scrotal hematoma,incision suture reaction,etc.)in the observation group(6.98%)was lower than that in the control group(23.26%),the difference was statistically significant(P<0.05).Conclusion:Single port LH surgery has shown good short-term results in the treatment of children with IH,with fewer complications,and can effectively alleviate the body's stress response and promote testicular development.
