探究淋巴细胞亚群在小儿原发免疫性血小板减少症中的临床意义

摘  要:目的旨在探讨原发免疫性血小板减少症(ITP)患儿T、B、NK细胞的百分比和绝对计数水平的表达情况,研究外周血淋巴细胞亚群的变化对ITP进展的影响及作用机制。方法选取2020年1月至2023年12月内蒙古自治区人民医院临床诊断为初治ITP的患儿48例为病例组(ITP组)和健康儿童30例为对照组。采用流式细胞术检测上述两组的淋巴细胞亚群,并对外周血中CD3^(+)、CD3^(+)CD4^(+)、CD3^(+)CD8^(+)、CD3^(+)CD4^(+)CD8^(+)T细胞,CD3^(-)CD19^(+)B细胞,CD3^(-)CD16^(+)CD56^(+)NK细胞的百分比和绝对计数水平及CD4^(+)/CD8^(+)值和T、B、NK总体的变化情况进行测定。结果ITP组和对照组进行对比,ITP组CD3^(+)T细胞、CD3^(+)CD4^(+)CD8^(+)T细胞、CD3^(+)CD8^(+)T细胞百分比,CD3^(-)CD19^(+)B细胞百分比和绝对计数高于对照组;CD3^(-)CD16^(+)CD56^(+)NK细胞百分比和绝对计数均低于对照组;淋巴细胞亚群变化总百分比减低,差异具有统计学意义(P<0.001)。两组CD3^(+)T细胞、CD3^(+)CD8^(+)T细胞绝对计数,CD3^(+)CD4^(+)T细胞百分比和绝对计数及CD4^(+)/CD8^(+)值差异无统计学意义(P>0.05)。结论ITP患儿存在免疫紊乱的现象,ITP的发病机制可能与NK细胞降低有关,或许和患儿的疾病进展相关。Objective To evaluate the expression of percentages and absolute count levels of lymphocyte subsets in children with immune thrombocytopenia(ITP),and to study the impact of changes in peripheral blood lymphocyte subsets on the progression of immune thrombocytopenia(ITP).Influence.Method Select 48 children with clinically diagnosed ITP from the People's Hospital of Inner Mongolia Autonomous Region from January 2020 to December 2023 as the ITP case group and 30 healthy children as the control group,and use flow cytometry to detect the lymphocytes of the above two groups.Cell subsets,and the percentage and absolute count levels of CD3^(+),CD3^(+)CD4^(+),CD3^(+)CD8^(+)T cells,CD3^(+)CD4^(+)CD8^(+),CD3^(-)CD19^(+)B cells,CD3^(-)CD16^(+)CD56^(+)NK cells and CD4^(+)/CD8^(+)value and overall changes in T,B,and NK were measured.Results Comparing the ITP group with the normal group,the ITP group had higher CD3^(+)T cells,CD3^(+)CD4^(+)CD8^(+)T cells,CD3^(+)CD8^(+)T percentage,CD3^(-)CD19^(+)B cell percentage and absolute count than the normal group(P<0.05),the percentage and absolute count of CD3^(-)CD16^(+)CD56^(+)NK cells were lower than those of normal children(P<0.001),and the total percentage of changes in lymphoid subgroups was reduced(P<0.001).The difference was statistically significant.CD3^(+)T cells,CD3^(+)There was no statistically significant difference in the absolute count of CD8^(+)T cells,the percentage and absolute count of CD3^(+)CD4^(+)T cells,and the CD4^(+)/CD8^(+)value(P>0.05).Conclusion Immunological disorders are present in children with ITP,and the pathogenesis of NK cells in ITP deserves further investigation.
