摘　　要：目的了解妊娠期糖尿病(GDM)患者弗林蛋白酶(Furin)、肿瘤生长因子-β(TGF-β)、血管内皮生长因子(VEGF)、稳态模型胰岛素抵抗指数(HOMA-IR)及丝氨酸蛋白酶抑制剂B1(SerpinB1)水平及Furin基因P1启动区r2071410 C/T位点多态性,并探讨其与深圳地区GDM的易感性。方法选择2021年10月至2024年3月GDM确诊患者116例(GDM组),年龄21~45岁,平均年龄28.23岁;孕前身体质量指数18.54~26.13 kg/m^(2),平均身体质量指数21.66 kg/m^(2);产次0~2次,平均产次0.34次;采血孕周24~26周,平均采血孕周24.73周。同期口服葡萄糖耐量试验正常孕妇89例(对照组),年龄20~46岁,年龄28.71岁;孕前身体质量指数19.82~27.34 kg/m^(2),平均身体质量指数21.86 kg/m^(2);产次0~3次,平均产次0.32次;采血孕周24~27周,平均采血孕周25.06周。采用酶联免疫吸附分析法检测Furin、TGF-β、VEGF及SerpinB1水平,并采用葡萄糖氧化酶法和化学发光免疫法测定空腹血糖(FPG)和胰岛素(Ins)水平,计算HOMA-IR。同时用反转录-实时荧光定量聚合酶链式反应法分析Furin基因P1启动区r2071410 C/T位点多态性。结果GDM组Furin、TGF-β、VEGF及HOMA-IR水平[(183.39±56.17)pg/mL、(25.05±5.46)ng/L、(29.30±6.21)ng/L及3.65±0.76]明显高于对照组[(61.42±15.83)pg/mL、(12.49±2.52)ng/L、(11.52±3.28)ng/L及1.82±0.34],而SerpinB1水平[(41.38±9.35)μg/L]明显低于对照组[(72.47±15.69)μg/L],差异有统计学意义(P<0.05)。经Pearson相关分析,Furin与TGF-β、VEGF及HOMA-IR水平呈正相关,而与SerpinB1水平呈负相关(r=0.6402、0.7154、0.7826、-0.7163,P<0.05);GDM组TT基因型和T等位基因频率分别为50.86%和60.78%,明显高于对照组8.99%和19.10%,而CC基因型和C等位基因频率分别为29.31%和39.22%,明显低于对照组70.79%和80.90%,差异有统计学意义(P<0.05);但CT基因型(19.83%vs 20.22%)差异无统计学意义(P>0.05);不同基因型GDM患者Furin水平差异有显著统计学意义(P<0.001),其中TT基因型Furin水平(Objective To investigate the levels of Furin protease(Furin),tumor growth factor-β(TGF-β),vascular endothelial growth factor(VEGF),homeostasis model insulin resistance index(HOMA-IR),serine protease inhibitor B1(SerpinB1)and r2071410 C/T locus polymorphism of Furin gene P1 promoter region in gestational diabetes mellitus(GDM),and investigate its relationship with the susceptibility of GDM in Shenzhen.Methods From October 2021 to March 2024,a total of 116 patients with GDM(GDM group)were enrolled,which aged 21-45 years old with mean age of 28.23 years old;pre-pregnancy body mass index(IBM)was 18.54-26.13 kg/m^(2) with mean BMI of 21.66 kg/m^(2);birth number was 0-2 with mean of 0.34;gestational age of blood collection was 24-26 weeks with mean of 24.73 weeks.At the same time,89 pregnant women with normal oral glucose tolerance test were set as control group),which aged 20-46 years old with mean age of 28.71 years old;pre-pregnancy BMI was 19.82-27.34 kg/m^(2) with mean BMI of 21.86 kg/m^(2);birth number was 0-3 with mean of 0.32;gestational age of blood collection was 24-27 weeks with mean of 25.06 weeks.The levels of Furin,TGF-β,VEGF and SerpinB1 were detected by enzyme-linked immunosorbent assay,the levels of fasting plasma glucose(FPG)and insulin(Ins)were measured by glucose oxidase method and chemiluminescence immunoassay,and HOMA-IR was calculated.The r2071410 C/T locus polymorphism of Furin gene P1 promoter region was analyzed by reverse transcription-real-time fluorescence quantitative polymerase chain reaction(RT-PCR).Results The levels of Furin,TGF-β,VEGF and HOMA-IR in GDM group[(183.39±56.17)pg/mL,(25.05±5.46)ng/L,(29.30±6.21)ng/L and 3.65±0.76]were significantly higher than those in control group[(61.42±15.83)pg/mL,(12.49±2.52)ng/L,(11.52±3.28)ng/L and 1.82±0.34].The level of SerpinB1[(41.38±9.35)μg/L]was significantly lower than that of the control group[(72.47±15.69)μg/L],the difference was statistically significant(P<0.05).The Pearson correlation analysis showed that Furin was positi
