摘　　要：泄泻是脾胃病科常见的疾病之一,其临床特征为频繁排便、粪便稀薄,严重时可呈水样泻。泄泻主要与体质虚弱、外邪侵袭或饮食不洁有关,这些因素均会导致脾胃功能减弱和脾肾阳气不足,进而出现排便次数的增多和排便形态的改变。李廷荃教授认为脾胃虚寒为发病之根源,情志不畅、肝气犯脾及饮食失宜皆为其病理因素,寒湿与热邪交错为标,肝、脾、胃三脏失调为本,乃为辨治之难点所在。临证时将泄泻分为寒热错杂型、脾肾阳虚型、肝郁脾虚型,治疗上强调以“调脾胃、安五脏、致中和“”正本清源”为基本法则,临床疗效显著。Diarrhea is a common disease in the department of spleen and stomach disorders,characterized by frequent defecation and loose or watery stools in severe cases.The condition is mainly associated with weak constitution,external pathogenic invasion,or unclean diet,all of which impair spleen and stomach function and lead to insufficiency of spleen and kidney yang,resulting in increased frequency and altered stool consistency.Professor LI Tingquan believes that spleen and stomach deficiency with cold is the root cause of the disease.Emotional distress,liver qi attacking the spleen,and dietary indiscretion are pathological factors.The intermingling of cold-dampness and heat pathogens represents the symptoms,while the imbalance of the liver,spleen,and stomach constitutes the fundamental cause,making treatment challenging.In clinical practice,diarrhea is categorized into mixed cold-heat type,spleen-kidney yang deficiency type,and liver qi stagnation with spleen deficiency type.The treatment emphasizes fundamental principles such as"regulating the spleen and stomach,stabilizing the five organs,and achieving harmony"and"rectifying the root and clearing the source."The approach has demonstrated significant clinical efficacy.
