摘　　要：目的:探究64排宝石能谱CT在心肌灌注成像中的应用。方法:选取2021年8月~2024年2月于本院就诊的80例心肌灌注成像患者,所有患者冠状动脉CT血管成像检查结果均为阳性,分析患者的临床资料,依据心肌灌注成像检查结果将患者分为心肌缺血阳性及心肌缺血阴性两组,对比两组基本情况与诊断结果。结果:女性在非心肌缺血组占比显著高于心肌缺血组(P<0.05),高血脂者在心肌缺血组中占比显著高于非心肌缺血组(P<0.05);在不同冠脉狭窄程度分级中,心肌缺血组中度狭窄(≥50%)占比显著低于非心肌缺血组(P<0.05),重度狭窄占比显著高于非心肌缺血组,在不同高危斑块类别中,心肌缺血组正性重构、餐巾环征及具有任意3个及以上高危斑块特征占比显著高于非心肌缺血组(P<0.05);两组斑块体积、斑块长度、直径狭窄度相比具有统计学差异(P<0.05)。结论:64排宝石能谱CT在心肌灌注成像中具有重要的预测价值,尤其是显著狭窄(≥70%)联合高危斑块对心肌缺血进行预测能够获得较高的准确率。Objective:To explore the application of 64-row gemstone energy spectrum CT in myocardial perfusion imaging.Methods:80 patients with myocardial perfusion imaging in our hospital from August 2021 to February 2024 were selected,and all patients had positive coronary CT angiography results.Clinical data of the patients were analyzed,and based on the results of myocardial perfusion imaging,they were divided into two groups:positive and negative for myocardial ischemia.The basic situation and diagnostic results of the two groups were compared.Results:The proportion of women in the non myocardial ischemia group was significantly higher than that in the myocardial ischemia group(P<0.05),and the proportion of hyperlipidemia patients in the myocardial ischemia group was significantly higher than that in the non myocardial ischemia group(P<0.05);In different grades of coronary artery stenosis,the proportion of moderate stenosis(≥50%)in the myocardial ischemia group was significantly lower than that in the non myocardial ischemia group(P<0.05),and the proportion of severe stenosis was significantly higher than that in the non myocardial ischemia group.In different high-risk plaque categories,the proportion of positive remodeling,napkin ring sign,and any three or more high-risk plaque features in the myocardial ischemia group was significantly higher than that in the non myocardial ischemia group(P<0.05);There were statistically significant differences(P<0.05)in plaque volume,plaque length,and diameter stenosis between the two groups.Conclusion:64-row gemstone energy spectrum CT has important predictive value in myocardial perfusion imaging,especially in predicting myocardial ischemia with high accuracy when combined with significant stenosis(≥70%)and high-risk plaques.
