运用能谱CT分析食管胃底静脉曲张程度与门静脉及脾静脉宽度关系

摘　　要：目的:运用能谱CT分析食管胃底静脉曲张程度与门静脉及脾静脉宽度的关系。方法:选择2019年2月~2024年3月本院收治的100例肝硬化患者分别进行胃镜检查及能谱CT对于腹部进行三期扫描。根据胃镜检查结果对于食管胃底静脉曲张分成Ⅰ度、Ⅱ度、Ⅲ度,与能谱CT检查测量的脾静脉宽度进行比较。结果:参照胃镜检查三种静脉曲张能谱CT增强检查的准确率分别为:单纯食管静脉曲张患者准确率92.68%;单纯胃底静脉曲张患者准确率91.67%;食管胃底静脉曲张患者准确率94.29%。两组检查方法结果比较不具有统计学意义(P>0.05)。根据能谱CT检查测量的93例肝硬化患者的门静脉及脾静脉内径宽度值,得出三组之间脾静脉的宽度存在明显差异,均有统计学意义(P<0.05)。并且,随着脾静脉宽度的增加静脉曲张程度亦越严重。结论:肝硬化引起的食管胃底静脉曲张程度可以通过门静脉及脾静脉宽度进行评价,亦可间接地评估门静脉压力高低。Objective:To analyze the relationship between the width of portal vein and splenic vein and the degree of esophageal and gastric fundus varices by energy spectrum CT.Methods:From February 2019 to March 2024,100 cases of cirrhosis of liver in our hospital were examined by gastroscopy and energy spectral CT.According to the results of gastroscopy,the gastroesophageal varices were divided intoⅠ,ⅡandⅢdegrees,and the splenic vein width measured by energy spectrum CT was compared.Results:According to gastroscopy,the accuracy rate of three kinds of ESG CT enhanced examination were as follows:the accuracy rate of simple esophageal varices(92.68%);The accuracy of gastric varices was 91.67%;The accuracy rate in patients with esophageal and gastric varices was 94.29%.The results of the two groups were not significant(P>0.05).According to 93 cases of cirrhosis of liver examined by spectral CT,the results showed that the widths of splenic veins were significantly different among the three groups(P<0.05).In addition,the degree of varicose veins increased with the increase of splenic vein width.Conclusion:The degree of esophagogastric varices caused by cirrhosis of liver can be evaluated by the widths of portal and splenic veins.
