摘 要:目的:评价光谱CT虚拟单能级联合骨科去金属伪影(O-MAR)技术在腰椎内固定术后的临床效果。方法:2023年1月~2023年12月选择在本院完成腰椎内固定的术后患者81例作为研究对象,所有患者均行常规CT扫描(对照组)、后期重建O-MAR图像(观察1组)、O-MAR扫描+光谱CT虚拟单能级200 KeV(观察2组),记录扫描图像主观与客观质量。结果:观察2组、观察1组、常规的图像质量主观优良率分别为100.00%,95.06%、90.12%,三组对比差异有统计学意义(P<0.05);观察2组、观察1组的椎弓根、椎体、侧隐窝、神经根、椎间盘、硬膜囊等腰椎部位的CT值显著低于对照组(P<0.05),观察2组也显著低于观察1组(P<0.05);观察2组、观察1组的椎弓根、椎体、侧隐窝、神经根、椎间盘、硬膜囊等腰椎部位的伪影指数也与对照组相比显著降低(P<0.05),观察2组与观察1组相比也显著降低(P<0.05)。结论:光谱CT虚拟单能级联合O-MAR在腰椎内固定术后的临床应用可提高图像质量主观优良率,可降低腰椎部位的CT与伪影指数,从而有效减少患者术后的金属伪影,为临床提供更多更准确的影像信息。Objective:To evaluate the clinical effect of spectral CT virtual single level combined with O-MAR after lumbar spine internal fixation surgery.Methods:From January 2023 to December 2023,81 postoperative patients who underwent lumbar internal fixation in our hospital were selected as the study subjects.All patients underwent routine CT scans(control group),post reconstruction O-MAR images(observation group 1),O-MAR scans+spectral CT virtual single level 200 KeV(observation group 2),and the subjective and objective quality of the scanned images were recorded.Results:The subjective excellent and good rates of image quality in observation group 2,observation group 1,and conventional group were 100.00%,95.06%,and 90.12%,respectively,and the difference between the three groups was statistically significant(P<0.05);The CT values of lumbar vertebrae such as pedicle,vertebral body,lateral recess,nerve root,intervertebral disc,and dural sac in observation group 2 and observation group 1 were significantly lower than those in the control group(P<0.05),and observation group 2 was also significantly lower than observation group 1(P<0.05);The pseudo index of lumbar vertebrae such as pedicle,vertebral body,lateral recess,nerve root,intervertebral disc,and dural sac in observation group 2 and observation group 1 were significantly reduced compared with the control group(P<0.05),and the pseudo index of lumbar vertebrae in observation group 2 was also significantly reduced compared with observation group 1(P<0.05).Conclusion:The clinical application of spectral CT virtual single level combined with O-MAR in lumbar spine internal fixation surgery can improve the subjective goodness rate of image quality,reduce the CT and artifact index of the lumbar spine,effectively reduce postoperative metal artifacts in patients,and provide more accurate imaging information for clinical practice.
