安魂汤加减联合佐匹克隆治疗老年血虚痰扰型失眠症合并情绪障碍的疗效

摘　　要：目的观察安魂汤加减联合佐匹克隆治疗老年血虚痰扰型失眠症合并情绪障碍的疗效。方法选取2023年3月—2024年9月于北京市鼓楼中医医院就诊的老年血虚痰扰型失眠症合并情绪障碍患者72例,采用随机数字表法分为观察组和对照组,每组36例。对照组采用佐匹克隆治疗,观察组在对照组基础上采用安魂汤加减治疗,2组均治疗4周。比较2组临床疗效,治疗前后睡眠质量[匹兹堡睡眠质量指数(PSQI)评分]、负面情绪[焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分]、多导睡眠监测参数[睡眠潜伏期(SL)、觉醒时间(AWT)、觉醒次数(AT)、快速眼动睡眠潜伏期(RL)、睡眠效率(SE)]及不良反应。结果观察组总有效率高于对照组(88.89%vs.66.67%,χ^(2)=5.143,P=0.023)。治疗4周后,2组PSQI、SAS和SDS评分均较治疗前下降,且观察组低于对照组(P<0.05或P<0.01);2组SL、AWT、AT、RL及SE均较治疗前改善,且观察组改善幅度大于对照组(P<0.01)。观察组与对照组不良反应总发生率比较差异无统计学意义(11.11%vs.5.56%,χ^(2)=0.182,P=0.670)。结论安魂汤加减联合佐匹克隆治疗老年血虚痰扰型失眠症合并情绪障碍可提高临床疗效,改善患者睡眠质量和负面情绪,调节多导睡眠监测参数,且安全性较高。Objective To observe the efficacy of modified Anhun decoction combined with zopiclone in treating insomnia with blood deficiency and phlegm disturbance syndrome in the elderly,accompanied by emotional disorders.Methods A total of 72 elderly patients with insomnia with blood deficiency and phlegm disturbance syndrome complicated with mood disorder who were treated in Beijing Gulou Hospital of Traditional Chinese Medicine from March 2023 to September 2024 were selected,they were divided into observation group and control group by random number table method,with 36 cases in each group.The control group was treated with zopiclone,and the observation group was treated with Anhun decoction modified on the basis of the control group.Both groups were treated for 4 weeks.The clinical efficacy,sleep quality(PSQI score),negative mood(SAS score,SDS score),polysomnatic monitoring parameters(SL,AWT,AT,RL,SE)before and after treatment,and adverse reactions of the 2 groups were compared.Results The total effective rate of observation group was higher than that of control group(88.89%vs.66.67%,χ^(2)=5.143,P=0.023).After 4 weeks of treatment,PSQI,SAS and SDS scores in 2 groups were decreased compared with those before treatment,and the scores in observation group were lower than those in control group(P<0.05 or P<0.01).SL,AWT,AT,RL and SE in 2 groups were improved compared with before treatment,and the improvement range in observation group was greater than that in control group(P<0.01).There was no significant difference in the total incidence of adverse reactions between observation group and control group(11.11%vs.5.56%,χ^(2)=0.182,P=0.670).Conclusion Anhun decoction modified combined with zopiclone in the treatment of insomnia with blood deficiency and phlegm disturbance syndrome complicated with mood disorders can improve clinical efficacy,improve patients′sleep quality and negative emotions,regulate polysomnia monitoring parameters,and have high safety.
