肺炎支原体23S rRNA耐药基因与肺炎支原体肺炎患儿肺功能的相关性

摘  要:目的探讨23S rRNA耐药基因与肺炎支原体肺炎(MPP)患儿肺功能、临床特征的相关性。方法选择2022年1月至2023年10月沧州市人民医院儿科收治的85例MPP患儿为研究对象。根据23S rRNA耐药基因检测结果将患儿分为阳性组(n=43)和阴性组(n=42)。比较2组患儿的临床症状、体征、住院时间、并发症、影像学特征及肺功能指标,并分析患儿肺功能指标与23S rRNA耐药基因的相关性。结果85例MPP患儿肺炎支原体23S rRNA耐药基因阳性者43例,阳性率为50.59%。2组患儿咳嗽、发热、气促及寒战发生情况比较差异无统计学意义(χ^(2)=1.036、0.903、0.567、0.171,P>0.05);阳性组患儿呼吸音减弱占比显著高于阴性组(χ^(2)=11.318,P<0.05)。阳性组患儿的住院时间显著长于阴性组(P<0.05)。阳性组患儿神经系统、消化系统并发症发生率显著高于阴性组(χ^(2)=4.814、4.9300x09,P<0.05);2组患儿心血管系统、血液系统及泌尿系统并发症发生率比较差异无统计学意义(χ^(2)=0.668、1.325、0.188,P>0.05)。阳性组患儿肺部病变的影像学特征肺不张、胸腔积液及肺实变率显著高于阴性组(χ^(2)=10.577、7.043、15.568,P<0.05);2组患儿肺坏死率比较差异无统计学意义(χ^(2)=1.001,P>0.05)。阳性组患儿肺功能指标潮气量(VT)、呼气峰流速时间占呼气时间比值(tPTEF/TE)及呼出75%潮气量时的瞬间流速与潮气呼气峰流速比(TEF25/PTEF)显著低于阴性组(P<0.05)。相关性分析结果显示,23S rRNA耐药基因阳性与MPP患儿肺功能指标VT、tPTEF/TE及TEF25/PTEF呈负相关(r=-0.571、-0.592、-0.520,P<0.05)。结论23S rRNA耐药基因阳性与MPP患儿肺功能呈负相关,耐药基因阳性可能会加重MPP患儿的肺功能损害,延长病程。Objective To analyze the correlation between 23S rRNA resistance gene and pulmonary function changes,clinical characteristics in children with Mycoplasma pneumoniae pneumonia(MPP).Methods A total of 85 children with MPP admitted to the Department of Pediatrics of People′s Hospital of Cangzhou City from January 2022 to October 2023 were selected as the research subjects.According to the detection results of 23S rRNA resistance genes,these children were divided into the positive group(n=43)and the negative group(n=42).The clinical symptoms,physical signs,length of hospital stay,complications,imaging features,and pulmonary function indicators were compared between the MPP children in the two groups,and the correlation between pulmonary function indicators and 23S rRNA resistance genes was analyzed.Results Among 85 children with MPP,43 were positive for MP 23S rRNA resistance genes,with a positive rate of 50.59%.There was no statistically significant difference in the incidence of cough,fever,shortness of breath,and chills between the two groups(χ^(2)=1.036,0.903,0.567,and 0.171,P>0.05).The proportion of respiratory sound weakening in the positive group was higher than that in the negative group(χ^(2)=11.318,P<0.05).The length of hospital stay in the positive group was significantly longer than that in the negative group(P<0.05).The incidence of neurological and digestive complications in the positive group was significantly higher than that in the negative group(χ^(2)=4.814 and 4.930,P<0.05).There was no statistically significant difference in the incidence of cardiovascular,hematologic,and urologic complications between the two groups(χ^(2)=0.668,1.325,and 0.188,P>0.05).The incidence of atelectasis,pleural effusion,and lung consolidation from the imaging features in the positive group was significantly higher than that in the negative group(χ^(2)=10.577,7.043,and 15.568,P<0.05).There was no statistically significant difference in the incidence of pulmonary necrosis between the two groups(χ^(2)=1.001,P>0.05)
