摘 要:目的探索动态增强磁共振成像(DCE-MRI)联合血清微小RNA-495-3p(miR-495-3p)、半胱氨酸蛋白酶8(CASP8)诊断前列腺癌的临床价值。方法选取2022年4月至2023年4月于湖北省中西医结合医院放射科就诊的前列腺癌患者87例作为前列腺癌组,另选同期良性前列腺增生(BPH)患者61例作为BPH组,所有患者均进行DCEMRI检查。在TargetScanHuman网站上进行miR-495-3p靶向CASP8生物信息学分析,比较分析两组患者血清miR-495-3p、CASP8水平,并采用受试者操作特征(ROC)曲线分析血清miR-495-3p、CASP8水平对前列腺癌的诊断价值。结果前列腺癌组患者血清miR-495-3p低于BPH组,CASP8水平高于BPH组,差异有统计学意义(P<0.05)。Target Scan Human网站预测结果显示,miR-495-3p与CASP8可能存在靶向关系。DCE-MRI共诊断出前列腺癌患者60例,BPH患者49例。ROC曲线分析结果显示,DCE-MRI联合血清miR-495-3p、CASP8水平早期诊断前列腺癌的曲线下面积显著高于DCE-MRI、miR-495-3p、CASP8单独诊断(P<0.05),敏感度、准确度和阴性预测值均高于DCE-MRI、miR-495-3p、CASP8单独诊断(P<0.05)。结论DCE-MRI联合血清miR-495-3p、CASP8水平能显著提高前列腺癌诊断的敏感度和准确度,对临床诊断前列腺癌具有重要价值。Objective To explore the clinical application value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)combined with serum microRNA-495-3p(miR-495-3p)and Caspase 8(CASP8)in the diagnosis of prostate cancer(PCa).Method From April 2022 to April 2023,87 PCa patients and 61 patients with benign prostatic hyperplasia(BPH)who visited Department of Radiology,Hubei Provincial Hospital of Integrated Chinese&Western Medicine were selected,all patients underwent DCE-MRI.Bioinformatics analysis of miR-495-3p targeting CASP8 was performed on the website Target Scan Human,and serum levels of miR-495-3p and CASP8 were compared and analyzed between the two groups.The diagnostic value of serum miR-495-3p and CASP8 levels in PCa was analyzed by receiver operator characteristic(ROC)curve.Result The serum miR-495-3p level in PCa group was lower than that in BPH group,and the CASP8 level was higher than that in BPH group(P<0.05).The prediction results of Target Scan Human website indicated that there may be a targeting relationship between miR-495-3p and CASP8.A total of 60 patients with PCa and 49 patients with BPH were diagnosed by DCE-MRI.ROC curve analysis results showed that the area under the curve of DCE-MRI combined with serum miR-495-3p and CASP8 levels in early diagnosis of PCa was significantly higher than that of three separate diagnosis(P<0.05).Sensitivity,accuracy and negative predictive value were higher than those of DCE-MRI,miR-495-3p and CASP8 alone(P<0.05).Conclusion The combination of DCEMRI and serum miR-495-3p and CASP8 levels can obviously improve the sensitivity and accuracy of PCa diagnosis,and has important value for clinical diagnosis of PCa.
