摘 要:目的探讨脂联素基因(ADIPOQ)rs2241766和rs1501299位点与血清脂联素及血脂异常之间的关联,为血脂异常与脂联素基因多态性的关联研究提供证据参考。方法于2017年4—6月在盐城市盐都区采用多阶段分层随机抽样法,获得资料完整的18岁以及上常住居民1400人,采用性别、年龄1∶1匹配的病例对照设计,共506名血脂异常病例获得匹配对照,最终纳入1012人。采用SAS 9.4软件进行方差分析、LSD-t检验和配对χ^(2)检验。哈迪-温伯格平衡(HWE)分析采用χ^(2)拟合优度检验。采用条件logistic回归模型评估rs2241766、rs1501299位点与血脂异常之间的关联强度,采用直线回归模型估计位点等位基因个数与血清脂联素水平的剂量反应关系。结果血脂异常病例组和对照组均为506例,男性均为260例,女性均为246例,平均年龄分别为(51.3±8.3)和(51.9±9.2)岁。对照组血清脂联素平均水平为(9.33±3.31)μg/ml,病例组为(8.60±3.39)μg/ml,两组脂联素水平差异有统计学意义(调整F=90.35,P<0.01)。两组rs2241766位点均是GG基因型人群脂联素水平最高,rs1501299位点均是TT基因型人群脂联素水平最高,均有统计学意义(LSD-t<0.01,P<0.05)。对照组和病例组rs2241766位点携带G等位基因个数(R2分别为0.03和0.04),rs1501299位点携带T等位基因个数(R2分别为0.03和0.02)均与血清脂联素水平均存在一定的剂量反应关系(P<0.01)。ADIPOQ基因rs1501299位点隐性模型中的GG基因型与血脂异常发生低风险相关(GG vs TG+TT,OR=0.58,95%CI:0.35~0.95,P<0.05)。不吸烟人群基因rs2241766位点加性模型G等位基因与血脂异常发生低风险相关(调整OR=0.72,95%CI:0.53~0.97,P<0.05)。结论ADIPOQ基因rs1501299位点GG基因型携带者与血脂异常发生低风险相关,rs2241766位点G等位基因与rs1501299位点T等位基因个数增加与高血清脂联素水平可能存在关联。Objective To explore the correlation between dyslipidemia and polymorphic site rs2241766,rs1501299 of ADIPOQ,and provide the evidence reference for the correlation between dyslipidemia and polymorphic adiponectin gene.Methods From April to June 2017,the multi-stage stratified random sampling was used to select 1400 residents(≥18 years old)with complete information in Yandu district of Yancheng city as the subjects,the 1∶1-matched case-control design for sex and age was used,a total of 506 dyslipidemia cases received matched controls and 1012 were finally included.Analysis of variance and LSD-t test,paired χ^(2) test were used to analyze the data.The used software was SAS 9.4.Hardy-Weinberg equilibrium(HWE)analysis was conducted using χ^(2) goodness-of-fit tests.The conditional logistic regression model was used to evaluate the correlation strength between rs2241766 and rs1501299 polymorphisms and dyslipidemia,the linear regression model was used to estimate the doseresponse relationship between the number of site alleles and serum adiponectin levels.Results The both dyslipidemia cases and control cases consisted of 506 individuals(260 males and 246 females),the mean age was(51.3±8.3)and(51.9±9.2)years old,respectively.The average serum adiponectin level(9.33±3.31μg/ml)in the control group was significantly higher than that(8.60±3.39μg/ml)in case group(P<0.01).The adiponectin levels were highest among individuals with the GG genotype at the rs2241766 locus in both groups,and highest among those with the TT genotype at the rs1501299 locus,with statistical significance(LSD-t values were all<0.01,and P values were all<0.05).In both control and case groups,the number of individuals carrying the G allele at the rs2241766 locus(R2 values were 0.03 and 0.04,respectively)and the number of individuals carrying the T allele at the rs1501299 locus(R2 values were 0.03 and 0.02,respectively)both showed a certain dose-response relationship with serum adiponectin levels(P<0.01).The GG genotype in the recessive model
