基于中西医临床病证特点的局灶节段性肾小球硬化症动物模型分析

摘  要:目的:随着社会环境的改变,全球局灶节段性肾小球硬化症的患病率不断增高,是当前发展为终末期肾病最主要的肾小球病因。因此,深入研究局灶节段性肾小球硬化症的病因病机,对该病的防治十分重要。笔者基于局灶节段性肾小球硬化症的中西医临床病证特点,结合目前动物模型的评价新方法,对该病的动物模型进行评价,给予分析和建议。方法:对中国知网、万方、维普及PubMed数据库进行“局灶节段性肾小球硬化症”和“动物模型”主题词检索,排除不符合条件的文献,对剩下的文献进行分类整理。基于局灶节段性肾小球硬化症的中西医临床病证特点、病因病机、中西医诊断标准,对该病的动物模型制备方法进行中西医吻合度分析。结果:模型吻合度最高的是阿霉素诱导模型和肾大部分切除术模型,且现有模型大多是以西医病机诊断为标准,与中医证型相关较少,总体西医吻合度较中医吻合度高。结论:局灶节段性肾小球硬化症的动物模型从中医致病因素、西医药物剂量、动物种类的选择上都存在一定不足,未来在模型中应更多地体现中医证候特点、统一药物剂量、探索其他动物品种模型,建立更加合适的病证结合模型,为中西医结合治疗局灶节段性肾小球硬化症提供更可靠的研究基础。Objective:As the social environment changes, focal segmental glomerulosclerosis(FSGS) shows increasing global prevalence and becomes the main glomerular cause of end-stage renal disease. Therefore, it is essential to study the etiology and pathogenesis of FSGS for the prevention and treatment of this disease. According to the characteristics of clinical symptoms and signs of FSGS in traditional Chinese and Western medicine, we evaluated the animal models of FSGS and put forward suggestions. Methods:The relevant articles were retrieved with the keywords of “FSGS” and “animal model” from China National Knowledge Infrastructure(CNKI),Wanfang Data, VIP,and PubMed and screened according to the inclusion and exclusion criteria. Based on the clinical characteristics, etiology, pathogenesis, and the diagnostic criteria in traditional Chinese and Western medicine, the modeling methods of FSGS were analyzed. Results:The adriamycin-induced model and subtotal nephrectomy model showed higher fitting degrees with the clinical characteristics. Most of the existing models were established based on the diagnosis of western medicine and rarely reflected TCM syndrome characteristics. The models showed higher fitting degrees with the clinical characteristics in Western medicine than with those in traditional Chinese medicine. Conclusion:The animal models of FSGS had shortcomings in the pathogenic factors of TCM,the drug dosage of Western medicine, and the choice of animal species. The future modeling should focus on the characteristics of TCM syndromes, unify the drug dosage, explore other animal species. More suitable animal models with disease-syndrome combination should be established to provide a reliable basis for the research on the treatment of FSGS with integrated traditional Chinese and Western medicine.
