吻合器痔上黏膜环切术治疗混合痔患者的效果

摘　　要：目的:观察吻合器痔上黏膜环切术(PPH)治疗混合痔患者的效果。方法:选取2021—2022年该院收治的90例混合痔患者进行前瞻性研究,按照随机数字表法将其分为观察组和对照组各45例。对照组行外剥内扎术(MMH)治疗,观察组行PPH治疗,比较两组临床指标(手术时间、创面愈合时间、住院时间)水平、术后不同时间疼痛[视觉模拟评分法(VAS)]评分、手术前后肛肠动力学指标[肛管收缩压(MSP)、肛管静息压(ARP)、肛管高压带长度]水平,以及并发症发生率。结果:观察组手术时间、住院时间、创面愈合时间均短于对照组,差异有统计学意义(P<0.05);术后1、3、7 d,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);术后1个月,两组MSP、ARP水平均高于术前,且观察组高于对照组,两组肛管高压带长度均短于术前,且观察组短于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为4.44%(2/45),低于对照组的22.22%(10/45),差异有统计学意义(P<0.05)。结论:PPH治疗混合痔患者可改善临床指标和肛肠动力学指标水平,降低术后不同时间疼痛评分和并发症发生率,效果优于MMH治疗。Objective:To observe effects of procedure for prolapse and hemorrhoids(PPH)in treatment of mixed hemorrhoids.Methods:A prospective study was conducted on 90 patients with mixed hemorrhoids admitted to this hospital from 2021 to 2022.According to the random number table method,they were divided into observation group and control group,45 cases in each group.The control group was treated with Milligan-Morgan hemorrhoidectomy(MMH),while the observation group was treated with PPH.The clinical indexes(operation time,wound healing time,hospitalization time)levels,the pain[visual analogue scale(VAS)]score at different time after the surgery,the anorectal dynamic indexes[anal systolic pressure(MSP),anal resting pressure(ARP)and anal high pressure zone length]levels before and after the surgery,and the incidence of complications were compared between the two groups.Results:The operation time,hospitalization time and wound healing time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).1,3 and 7 days after the surgery,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).1 month after the surgery,the levels of MSP and ARP in the two groups were higher than those before the surgery,and those in the observation group were higher than those in the control group;the length of anal high pressure zone in the two groups was shorter than that before the surgery,and that in the observation group was shorter than that in the control group;and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 4.44%(2/45),which was lower than 22.22%(10/45)in the control group,and the difference was statistically significant(P<0.05).Conclusions:PPH can improve the levels of clinical indexes and anorectal dynamics indexes of the patients with mixed hemorrhoids,and reduce the pain scores and the incidence of complicatio
