选择性会阴侧切与自然裂伤在自然分娩产妇中的应用效果比较

摘　　要：目的:比较选择性会阴侧切与自然裂伤在自然分娩产妇中的应用效果。方法:回顾性分析2021年5月至2023年5月于该院进行自然分娩的60名产妇的临床资料,按照助产方式不同将其分为研究组(n=30)和对照组(n=30)。研究组采用选择性会阴侧切,对照组采用自然裂伤,比较两组产后盆底肌功能分级、盆底肌肌电值、性功能[女性性功能指数(FSFI)]评分、肛提肌扩张情况和并发症发生率。结果:两组产后盆底肌功能分级比较,差异无统计学意义(P>0.05);两组慢肌(Ⅰ类纤维)阶段平均值、前静息阶段平均值、后静息阶段平均值比较,差异均无统计学意义(P>0.05);研究组快肌(Ⅱ类纤维)阶段最大值低于对照组,差异有统计学意义(P<0.05);研究组产后性唤起、性欲、性高潮、阴道润滑、性满意度、性交疼痛等FSFI评分均低于对照组,差异有统计学意义(P<0.05);研究组产后肛提肌扩张情况优于对照组,差异有统计学意义(P<0.05);研究组产后并发症发生率为30.00%(9/30),高于对照组的6.67%(2/30),差异有统计学意义(P<0.05)。结论:选择性会阴侧切应用于自然分娩产妇改善产后肛提肌扩张情况的效果优于自然裂伤,但会降低性功能、快肌(Ⅱ类纤维)阶段最大值,提高产后并发症发生率。Objective:To compare application effects of selective episiotomy and natural laceration in parturients with natural delivery.Methods:The clinical data of 60 parturients who underwent natural delivery in this hospital from May 2021 to May 2023 were retrospectively analyzed.According to different midwifery methods,they were divided into study group(n=30)and control group(n=30).The study group was treated with selective episiotomy,while the control group was treated with natural laceration.The postpartum pelvic floor muscle function classification,the pelvic floor muscle electromyography value,the sexual function[female sexual function index(FSFI)]score,the levator ani muscle expansion,and incidence of complications were compared between the two groups.Results:There was no significant difference in the postpartum pelvic floor muscle function classification between the two groups(P>0.05).There were no significant differences in the mean value of slow muscle(type I fiber)stage,the mean value of pre-resting stage and the mean value of post-resting stage between the two groups(P>0.05).The maximum value of the fast muscle(type II fiber)stage in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).The FSFI scores of postpartum sexual arousal,sexual desire,orgasm,vaginal lubrication,sexual satisfaction and sexual pain in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levator ani muscle expansion in the study group was better than that in the control group,and the difference was statistically significant(P<0.05).Further,the incidence of postpartum complications in the study group was 30.00%(9/30),which was higher than 6.67%(2/30)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Selective episiotomy is superior to natural laceration in improving of the levator ani muscle expansion rate in the parturients with natural delivery,but it can reduce the sexu
