摘　　要：目的观察助产士主导的精准孕期营养管理联合生育舞蹈对妊娠期糖尿病分娩结局的影响。方法回顾性选取2022年3月至2024年3月东莞市南城医院妊娠期糖尿病患者160例为研究对象,按照护理方式不同分为对照组(80例,常规护理和宣教)和观察组(80例,精准营养管理和指导生育舞蹈护理干预)。比较两组血糖控制情况、血糖达标率、自然分娩率以及分娩结局。结果两组干预前孕妇血糖水平比较,差异无统计学意义(P>0.05);干预后,两组空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组血糖达标率(87.50%)高于对照组(65.00%),观察组的自然分娩率(77.50%)高于对照组(67.50%),差异有统计学意义(P<0.05)。观察组早产、巨大儿、产后出血不良结局总发生率(3.75%)低于对照组(13.75%),差异有统计学意义(P<0.05)。结论助产士主导的精准孕期营养管理联合生育舞蹈可下调妊娠期糖尿病孕妇的血糖指标,提升血糖达标率,对降低不良分娩结局有积极作用,建议推广应用。Objective To observe the effect of midwife-led precise nutrition management during pregnancy combined with dancing for birth on the delivery outcome of gestational diabetes mellitus.Methods From March 2022 to March 2024,160 patients with gestational diabetes mellitus in Dongguan Nancheng Hospital were selected retrospectively.According to different nursing methods,they were divided into control group(80 cases,routine nursing and education)and observation group(80 cases,precise nutrition management and dance nursing intervention).The blood glucose control,blood glucose compliance rate,natural delivery rate and delivery outcome were compared between the two groups.Results There was no significant difference in blood glucose level between the two groups before intervention(P>0.05).After the intervention,the fasting plasma glucose(FPG),2 hour postprandial glucose(2hPG)and hemoglobin A1c(HbA1c)in the two groups were lower than those before the intervention,and the differences were statistically significant(P<0.05).The blood glucose rate in the observation group(87.50%)was higher than that in the control group(65.00%),and the natural delivery rate in the observation group(77.50%)was higher than that in the control group(67.50%),with a statistically significant difference(P<0.05).The total incidence of premature delivery,macrosomia and postpartum hemorrhage in the observation group(3.75%)was lower than that in the control group(13.75%),and the difference was statistically significant(P<0.05).Conclusion Midwife-led precise nutrition management during pregnancy combined with dancing for birth can reduce the blood glucose index of pregnant women with gestational diabetes mellitus,improve the blood glucose compliance rate,and have a positive effect on reducing adverse delivery outcomes,so it is recommended to be popularized and applied.
