摘 要:目的探究氯醋甲胆碱支气管激发试验在儿童支气管高反应性评估中的实用性和安全性。方法选取2023年4月至2024年2月六盘水市人民医院诊治的5~14岁支气管哮喘患儿80例,所有纳入研究的对象均接受肺功能检查、氯醋甲胆碱激发试验,对80例支气管哮喘患儿进行随机分组,分为肺功能组和氯醋甲胆碱组,所有患儿均接受系统治疗,肺功能组患儿以肺功能检查及呼出气一氧化氮指导糖皮质激素剂量调整,氯醋甲胆碱组患儿以基础肺功能及氯醋甲胆碱激发试验指导糖皮质激素剂量调整。治疗结束后,比较两组肺功能检查指标差异、肺功能变化和气道炎症水平指标。结果氯醋甲胆碱组呼气峰流速百分比、第1秒用力呼气容积占预计值百分比、用力肺活量百分比指标均高于肺功能组;最大呼气峰流速,25%、50%、75%呼气中期流量指标,呼出气一氧化氮检查,肺泡及肺泡区呼出气一氧化氮浓度指标均高于肺功能组,差异有统计学意义(P<0.05)。结论氯醋甲胆碱支气管激发试验在儿童支气管高反应性评估中实用性和安全性均较高,具有重要的价值和应用前景。Objective To investigate the practicability and safety of methacholine chloride(MCT)bronchial provocation test in the evaluation of bronchial hyperresponsiveness in children.Methods A total of 80 children with bronchial asthma aged 5-14 years admitted to and treated in the Peoples Hospital of Liupanshui City from April 2023 to February 2024 were selected,and all the subjects in the research were treated with pulmonary function examination and MCT provocation test,and the children with bronchial asthma were randomly divided into the pulmonary function group and the MCT group.All the children received systematic treatment.The children in the pulmonary function group were guided by pulmonary function examination and exhaled nitric oxide to adjust the dose of glucocorticoid,while the children in the MCT group were guided by basic pulmonary function examination and MCT provocation test to adjust the dose of glucocorticoid.After the treatment,the differences of pulmonary function indices,changes of pulmonary function and airway inflammation level indices were compared between the two groups.Results The indices of peak expiratory flow variability,the percentage of forced expiratory volume in the first second to the predicted value and forced vital capacity in the MCT group were all higher than those in the pulmonary function test group.The peak expiratory flow rate,mid-expiratory flow rate of 25%,50%and 75%,fractional exhaled nitric oxide and concentration of alveolar and alveolar region exhaled nitric oxide indices were all higher than those of pulmonary function examination group,with statistically significant differences(P<0.05).Conclusion MCT bronchial provocation test is very practical and safe in the evaluation of bronchial hyperresponsiveness in children,and it has important value and application prospect.
