摘  要:目的探讨磁共振(MR)动态增强时间-强度曲线(TIC)联合弥散加权成像(DWI)鉴别诊断乳腺病变的价值。方法选取福建省漳州市医院影像科2023年1—12月收治的乳腺病变患者274例为研究对象,均采取核磁共振成像(MRI)检查,对增强检查图像做TIC曲线分析。进行磁共振DWI检查获得ADC值,参照病理结果评价不同检查方式对乳腺病变价值,比较良恶性病变ADC值、TIC曲线。结果病理证实良性152例(55.47%)、恶性122例(44.53%)。联合诊断灵敏度98.36%、准确度97.08%均高于单独检查(P<0.05)。良性病变组ADC值高于恶性病变组(P<0.05);良性病变组的TIC曲线以Ⅰ型(57.89%)、Ⅱ型(32.89%)为主,恶性病变组TIC曲线以Ⅱ型(54.92%)、Ⅲ型(39.34%)为主。结论乳腺病变采用MRI动态增强TIC曲线联合DWI检查对于病变良恶性鉴别诊断的准确率高,且具有较高的诊断灵敏度。Objective To explore the value of magnetic resonance(MR)dynamic enhanced time-intensity curve(TIC)combined with diffusion-weighted imaging(DWI)in the differential diagnosis of breast lesions.Methods A total of 274 patients with breast lesions admitted to the Imaging Department,Zhangzhou Municipal Hospital of Fujian Province from January to December 2023 were selected as the research subjects.All patients underwent magnetic resonance imaging(MRI)examination,and TIC curve analysis was performed on the enhanced images.DWI was performed to obtain ADC values,and the value of different examination methods for breast lesions was evaluated according to pathological results,and the ADC values and TIC curves of benign and malignant lesions were compared.Results Pathological examination confirmed 152 cases(55.47%)as benign and 122 cases(44.53%)as malignant.The sensitivity of combined diagnosis was 98.36%,and the accuracy was 97.08%,both of which were higher than those of individual examination(P<0.05).The ADC value in the benign lesion group was higher than that in the malignant lesion group(P<0.05).The TIC curves of the benign lesion group were mainly type I(57.89%)and typeⅡ(32.89%),while the TIC curves of the malignant lesion group were mainly typeⅡ(54.92%)and typeⅢ(39.34%).Conclusion The use of MRI dynamic enhanced TIC curve combined with DWI examination for the differential diagnosis of benign and malignant breast lesions has high accuracy and diagnostic sensitivity.
