摘  要:目的探究重症肺部感染患者外周血白细胞介素6(IL-6)、Toll样受体2(TLR-2)、白细胞分化抗原14(CD14)的表达水平,分析其对不同病原菌的鉴别价值。方法选取2021年9月至2023年8月江苏省海安市人民医院重症肺部感染患者91例为观察组、55名体检健康者为对照组。检测2组外周血IL-6、TLR-2、CD14表达水平(酶联免疫吸附法、化学发光法)并进行比较。比较不同病原菌感染患者外周血IL-6、TLR-2、CD14表达水平,采用受试者工作特征(ROC)曲线分析重症肺部感染患者外周血IL-6、TLR-2、CD14水平对不同病原菌的鉴别价值。结果观察组外周血IL-6、TLR-2和CD14水平均高于对照组(P<0.05)。在外周血IL-6水平方面,G^(-)菌、真菌、G^(+)菌感染者依次降低,且两两比较差异显著(均P<0.05);在外周血TLR-2水平方面,G^(-)菌感染者低于G^(+)菌和真菌感染者(P<0.05),G^(+)菌感染者与真菌感染者比较差异无统计学意义(P>0.05);在外周血CD14水平方面,G^(-)菌、G^(+)菌、真菌感染者比较差异无统计学意义(P>0.05)。ROC曲线分析结果显示,鉴别G^(-)菌、真菌时,IL-6、TLR-2联合的曲线下面积(AUC)(0.880)大于IL-6(0.819)、TLR-2(0.750)单独鉴别;鉴别G^(-)菌、G^(+)菌时,IL-6、TLR-2联合鉴别的AUC(0.885)大于IL-6(0.820)、TLR-2(0.783)单独鉴别;IL-6单独鉴别G^(+)菌、真菌的AUC为0.749。结论重症肺部感染患者外周血IL-6、TLR-2、CD14表达异常升高,其中IL-6、TLR-2可用于不同病原菌类型的鉴别。Objective To explore the expression levels of interleukin-6(IL-6),Toll-like receptor 2(TLR-2),and leukocyte differentiation antigen-14(CD14)in the peripheral blood of patients with severe pulmonary infection,and analyze their identification value for different pathogenic bacteria.Methods 91 patients(severe pulmonary infection)admitted to o Haian people's hospital(September 2021 to August 2023)were selected as observation group,during the same period,55 healthy persons(underwent physical examination)were selected as control group.The expression levels of IL-6,TLR-2,and CD14 in the peripheral blood of the two groups were detected by enzyme-linked immunoassay,chemiluminescence method.The levels were compared among different pathogens,and receiver operating characteristic(ROC)curves were used to analyze the identify identification value of peripheral blood IL-6,TLR-2,and CD14 expression in patients with severe pulmonary infection for different pathogenic bacteria.Results The levels of peripheral blood IL-6,TLR-2,and CD14 in the observation group were higher than control group(P>0.05).In terms of IL-6 level in peripheral blood,G^(-)bacteria,fungi and G^(+)bacteria infected patients were decreased successively,and ptwo comparison differences were significant(all P<0.05).In terms of TLR-2 level in peripheral blood,G^(-)bacteria infected patients were lower than that of G^(+)bacteria and fungi infected patients(P<0.05),and there was no statistical significance between G^(+)bacteria infected patients and fungi infected patients(P>0.05).There was no significant difference in the level of CD14 in peripheral blood among G^(-)bacteria,G^(+)bacteria and fungi(P>0.05).ROC curve analysis results showed that the combined area under the curve(AUC)value of IL-6 and TLR-2(0.880)was higher than that of IL-6(0.819)and TLR-2(0.750)in the identification of G^(-)bacteria and fungi.In the identification of G^(-)bacteria and G^(+)bacteria,the AUC value of IL-6 and TLR-2 combined identification(0.885)was higher than that of IL-6(0.820)and T
