摘　　要：目的探讨固本降糖方联合地特胰岛素治疗妊娠糖尿病(GDM)患者的效果及对脂肪因子、炎症因子和母婴结局的影响。方法将泰安市妇幼保健院产科2023年1月至8月收治的98例GDM患者采用抽签法分为对照组(49例)和观察组(49例)。对照组采用地特胰岛素治疗,观察组在对照组的基础上采用固本降糖方治疗,比较两组血糖、脂肪因子、炎症因子水平以及母婴结局。结果与治疗前比较,两组治疗后的血糖水平均降低,且观察组的空腹血糖、餐后2 h血糖水平比对照组更低(P<0.05);治疗后观察组的超敏C反应蛋白、肿瘤坏死因子-α、白细胞介素-6水平、瘦素、内脂素水平低于对照组,脂联素水平高于对照组(P<0.05);两组不良母婴结局总发生率及剖宫产率比较,差异无统计学意义(P>0.05)。结论固本降糖方联合地特胰岛素能够更好地调节GDM患者的血糖水平和脂肪因子水平,减轻炎症反应。Objective To investigate the effect of Guben Jiangtang Recipe combined with Insulin Detemir in the treatment of patients with gestational diabetes mellitus(GDM)and its influence on adipokines,inflammatory factors and maternal and infant outcomes.Methods A total of 98 patients with GDM admitted to the Department of Obstetrics,Taian Maternity and Child Health Hospital from January to August 2023 were selected and divided into control group(49 cases)and observation group(49 cases)by lottery method.The control group was treated with Insulin Detemir,while the observation group was treated with Guben Jiangtang Recipe on the basis of control group.The levels of blood glucose,adipokines,inflammatory factors and maternal and infant outcomes were compared between the two groups.Results Compared with before treatment,the blood glucose levels in both groups were decreased after treatment,and the fasting blood glucose and 2 h postprandial blood glucose levels in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of hypersensitive C-reactive protein,tumor necrosis factor-α,interleukin-6,leptin and lactone in the observation group were lower than those in the control group,and the level of adiponectin was higher than that in the control group(P<0.05).There were no significant differences in total rate of adverse maternal and infant outcomes and cesarean section rate between the two groups(P>0.05).Conclusion Guben Jiangtang Recipe combined with Insulin Detemir can better regulate blood glucose and adipokine levels in patients with GDM,and alleviate inflammation.
