门诊处方前置审核系统对生殖中心用药合理化分析

摘　　要：目的 通过对南京医科大学附属苏州医院生殖中心处方前置审核系统提醒条目进行合理性分析,提高生殖中心用药的安全性与合理性。方法 采用回顾性分析,选取南京医科大学附属苏州医院生殖中心实施处方前置审核前(2022年1—6月)与实施处方前置审核后(2023年1—6月)的电子处方,比较实施处方前置审核前后用药合理性的差异。并对处方前置审核系统数据进行维护和完善。结果 生殖中心处方前置审核系统提醒条目适应证不适宜由31 137降为2 030条,禁忌诊断由1 427条降为61条,用量(剂量)异常的由7 179条降为91条,重复用药的由4 380降为36条。给药频率异常、给药途径异常、不适宜溶质等的提醒条目均有明显减少。禁忌人群由1条增加到4条。结论 生殖中心用药复杂,通过处方前置审核系统数据分析与维护,可提高生殖中心用药的安全性。Objective To improve the safety and rationality of drug use in the reproductive center by analyzing the rationality of the reminder items of the prescription pre-review system in the Reproductive Center of Suzhou Hospital Affiliated to Nanjing Medical University.Methods The retrospective analysis method was adopted.The electronic prescriptions of the Reproductive Center of Suzhou Hospital Affiliated to Nanjing Medical University before the implementation of prescription pre-review(from January to June 2022)and after the implementation of prescription pre-review(from January to June 2023)were selected to compare the differences in the rationality of drug use before and after the implementation of prescription pre-review.Meanwhile,the data of the prescription pre-review system were maintained and improved.Results In the Reproductive Center,the number of reminder items in the prescription pre-review system indicating inappropriate indications decreased from 31137 to 2030,the number of items related to contraindicated diagnoses decreased from 1427 to 61,the number of items indicating abnormal dosage(dose)decreased from 7179 to 91,and the number of items indicating repeated medication decreased from 4380 to 36.The number of reminder items related to abnormal dosing frequency,abnormal administration route,inappropriate solute,etc.also decreased significantly.The number of items related to contraindicated populations increased from 1 to 4.Conclusions The drug use in the reproductive center is complex.Through the data analysis and maintenance of the prescription pre-review system,the safety of drug use in the Reproductive Center can be improved.
