摘  要:目的:探讨解聚对单采血小板产品中血小板活化指标的影响,为输血安全和血液质量评估提供科学依据。方法:45例采集结束后肉眼可见聚集,并于24小时内完全解聚的单采血小板为实验组,同期45例无聚集的单采血小板产品为对照组,使用电子显微镜观察血小板形态,同时用流式细胞术和ELISA检测血小板活化情况。结果:解聚后电镜观察单采血小板体积形态无明显变化。CD62P在解聚后血小板产品中的表达率为(28.2±17.9)%,与无聚集的血小板产品的(16.6±9.8)%相比有明显增高,差异有统计学意义(P<0.05)。解聚后血小板产品中Annexin V阳性率为(55.3±19.3)%,血浆中PF4和β-TG含量分别为(10.4±6.9)ng/mL和(94.6±49.8)pg/mL,无聚集的血小板产品中Annexin V阳性率为(60.8±21.8)%,血浆中PF4和β-TG含量分别为(8.3±6.3)ng/mL和(96.7±63.2)pg/mL,两组对比差异无统计学意义(P>0.05)。结论:解聚后的单采血小板产品活化率有所增高,血小板凋亡比例和释放功能未见明显改变,临床上可根据解聚后血小板产品特性,结合患者病情,合理科学使用血小板。Objective:To investigate the activation indicators changes in disaggregated apheresis platelet products.Methods:45 products with aggregates which were completely disaggregated within 24 hours were observed,45 aggregate-free products were included in the control group simultaneously.The morphology of platelets was observed by electron microscope,and activation indicators of platelets were evaluated by flow cytometry and ELISA.Results:There were no significant changes in morphology of platelets after disaggregation observed by electron microscope.The expression of CD62P in the disaggregated products(28.2±17.9)%was obviously increased compared with the aggregate-free products(16.6±9.8)%,and the difference is statistically significant(P<0.05).The positive rate of Annexin V in the disaggregated products was(55.3±19.3)%,and the levels of PF4 andβ-TG in plasma were(10.4±6.9)ng/mL and(94.6±49.8)pg/mL,Compared with the aggregate-free products(60.8±21.8)%,(8.3±6.3)ng/mL,and(96.7±63.2)pg/mL,there was no statistically significant difference(P>0.05).Conclusion:The activation of platelets is improved,and the proportion of platelet apoptosis and release function are not obviously changed in disaggregated products.
