摘　　要：目的:探讨临床应用24h动态心电图(DCG)对冠心病(CHD)性心律失常进行检测的诊断价值。方法:选取2023年2月—2024年2月阳春市人民医院收治的180例CHD性心律失常患者,分别进行常规心电图(ECG)检查和24h DCG检查,比较两种心电图检查方式诊断结果、检出情况以及患者评分。结果:24h DCG的阳性检出率为72.78%,高于ECG的52.78%(x^(2)=15.406,P<0.05)。心房颤动、短阵室上性心动过速以及房性/室性期前收缩二三联律,房性/室性期前收缩成对的24h DCG的检出率高于ECG(P<0.05)。两种检查方式患者评分相比,24h DCG的实用性和准确性评分更高(P<0.05)。结论:24h DCG检查冠心病性心律失常的效能更好,尤其擅长识别心房颤动,房性/室性期前收缩成对,短阵室上性心动过速以及房性/室性期前收缩二三联律,且患者评分更高,值得临床应用推广。Objective:To evaluate the diagnostic value of 24h dynamic electrocardiogram(DCG)in the detection of coronaryheart disease(CHD)arrhythmia.Methods:180 cases of CHD arrhythmia admitted to ourhospital from February 2023 to February 2024 were selected,and routine electrocardiogram(ECG)examination and 24-h DCG examination were respectively performed.The diagnosis results,detection conditions and patient scores of the two ECG examination methods were compared.Results:The positive rate of DCG at 24h was 72.78%,higher than that of ECG(52.78%)(x^(2)=15.406,P<0.05).The detection rate of 24h DCG in atrial fibrillation,short paroxysmal supraventricular tachycardia and atrial/ventricular presystole washigher than that in ECG(P<0.05).Compared with the two examination methods,24h DCGhadhigher practicability and accuracy scores(P<0.05).Conclusion:24h DCG is more effective in detecting coronary arrhythmias,especially in identifying atrial fibrillation,atrial/ventricular presystolic pairs,short paroxysmal supraventricular tachycardia,and atrial/ventricular presystolic two-three combination,and it is worthy of clinical application and promotion.
