摘　　要：目的探究老年慢性心力衰竭患者(CHF)血清LncRNA-ZFAS1,miR-150-5p与心功能分级及预后的相关性研究。方法选取2021年1月至2022年6月于华北石油管理局总医院治疗的老年CHF患者100例,按照纽约心脏协会(NHYA)心功能分级分为Ⅱ级组(31例),Ⅲ级组(43例),Ⅳ级组(26例),并根据预后情况分为预后良好组(70例)与预后不良组(30例),另选同期本院体检的健康志愿者62例作为对照组。qRT-PCR检测血清LncRNA-ZFAS1,miR-150-5p的相对表达水平,并收集分析患者的临床资料,采用Pearson法分析LncRNA-ZFAS1与miR-150-5p之间与心功能指标的相关性,绘制受试者工作特征(ROC)曲线评估血LncRNA-ZFAS1,miR-150-5p对老年CHF患者预后的预测价值。结果与对照组相比,CHF患者的LncRNA-ZFAS1水平升高,miR-150-5p水平降低(P<0.05);CHF心功能Ⅱ级组、Ⅲ级组、Ⅳ级组患者的LncRNA-ZFAS,miR-150-5p水平之间差异具有统计学意义(P<0.05),预后不良组与预后良好组患者在是否有心脏病、心功能指标及LncRNA-ZFAS,miR-150-5p之间差异具有统计学意义(P<0.05);且CHF患者血清LncRNA-ZFAS1与左室射血分数(LVEF)、miR-150-5p呈负相关,与左心室舒张末期内经(LVEDD)、左心房内径(LAD)、左心室质量指数(LVMI)呈正相关(P<0.05),miR-150-5p与LVEF呈正相关,与LVEDD、LAD、LVMI呈负相关(P<0.05),LncRNAZFAS1,miR-150-5p以及联合的AUC分别为0.831、0.851、0.910,两者联合预测显著优于LncRNA-ZFAS1(Z=1.973,P=0.049),miR-150-5p(Z=2.067,P=0.039)单独预测。结论老年CHF患者LncRNAZFAS1水平升高,miR-150-5p水平降低,与心功能分级及预后具有相关性。Objective To investigate the correlation between serum LncRNA-ZFAS1,miR-150-5p,cardiac function grading,and prognosis in elderly patients with chronic heart failure(CHF).Methods From January 2021 to June 2022,100 elderly CHF patients who underwent treatment in General Hospital of North China Petroleum were collected,according to the New York Heart Association(NHYA)cardiac function classification,they were grouped into gradeⅡgroup(n=31),gradeⅢgroup(n=43),and gradeⅣgroup(n=26),they were separated into a good prognosis group(n=70)and a poor prognosis group(n=30)based on the prognosis,in addition,62 healthy volunteers who underwent physical examinations in our hospital were included as the reference group.QRT-PCR was applied to detect the relative expression levels of serum LncRNA-ZFAS1,miR-150-5p.Pearson method was applied to analyze the correlation between LncRNA-ZFAS1 and miR-150-5p,and their relationship with cardiac function indicators,receiver operating characteristic(ROC)curve was plotted to evaluate the predictive value of serum LncRNA-ZFAS1,miR-150-5p for the prognosis of elderly CHF patients.Results Compared with the reference group,the level of LncRNA-ZFAS1 in CHF patients was obviously increased,while the level of miR-150-5p was obviously decreased(P<0.05);The difference between the levels of LncRNA-ZFAS,miR-150-5p in patients in the CHF classⅡ,classⅢ,and classⅣgroups was statistically significant(P<0.05);the statistically difference between patients with or without heart disease,cardiac function indices,and LncRNA-ZFAS,miR-150-5p between patients in the poor prognosis group and the good prognosis group(P<0.05);the serum LncRNA-ZFAS1 level in CHF patients was negatively correlated with LVEF and miR-150-5p,but positively correlated with LVEDD,LAD,and LVMI(P<0.05),miR-150-5p was positively correlated with LVEF,negatively correlated with LVEDD,LAD,and LVMI(P<0.05);the AUC of LncRNA-ZFAS1,miR-150-5p,and their combination was 0.831,0.851,and 0.910,respectively,the combined prediction of the two was
