缺血性脑卒中患者复发恐惧的影响因素分析

摘　　要：目的了解缺血性脑卒中患者的复发恐惧现状并分析患者复发恐惧的影响因素,为开展情绪管理提供可靠的参考价值。方法纳入医院2023年1月~2024年8月收治90例符合纳排标准的住院缺血性脑卒中患者为研究样本,通过收集患者临床资料,并根据复发恐惧情况分为正常组(45例)、异常组(45例)。对比患者的建档资料(年龄、职业、文化程度、Barthel指数等)、主要照顾者、居住状态、病耻感等,分析缺血性脑卒中患者复发恐惧的影响因素,借助Logistic回归分析方法分析危险因素。结果缺血性脑卒中复发恐惧评分37.52±1.04分。恐惧组和正常组相比,文化程度、Barthel指数、身体质量指数(BMI)、复发次数、自我感受有无负担、居住状态、家庭是否亲密对比存在统计学差异(P<0.05)。经Logistic回归分析方法分析:Barthel指数降低、BMI异常、复发次数≥2次、自我感受有负担、独居状态及家庭不亲密是导致缺血性脑卒中患者恐惧复发的影响危险因素。结论Barthel指数降低、BMI异常、复发次数≥2次、自我感受有负担、独居状态及家庭不亲密均是导致缺血性脑卒中患者恐惧复发的影响因素,在临床工作中应加强对危险因素的及时干预,对降低其恐惧情绪具有重要意义。Objective To understand the current situation of relapse fear in patients with ischemic stroke and analyze the influencing factors of relapse fear in patients,so as to provide reliable reference value for emotional management.Methods A total of 90 inpatients with ischemic stroke who met the exclusion criteria and were admitted to the hospital during from January 2023 to August 2024 were included as study samples.Clinical data of the patients were collected and divided into normal group(45 cases)and abnormal group(45 cases)according to the recurrence fear.By comparing the documented data of patients(age,occupation,education level,Barthel index,etc.),main caregivers,residence status,stigma,etc.,the influencing factors of recurrence fear in patients with ischemic stroke were analyzed,and the risk factors were analyzed by Logistic regression analysis.Results Ischemic stroke recurrence fear score 37.52±1.04.Compared with the normal group,there were statistical differences in education level,Barthel index,body mass index(BMI),recurrence frequency,self-perceived burden,living status,and family closeness between the fear group and the normal group(P<0.05).Logistic regression analysis showed that lower Barthel index,abnormal BMI,recurrence≥2 times,self-feeling burden,solitary state and family inintimacy were the risk factors for recurrence fear in patients with ischemic stroke.Conclusion Decreased Barthel index,abnormal BMI,recurrence frequency≥2 times,self-feeling burden,solitary state and family intimacy are all factors that lead to recurrent fear in patients with ischemic stroke.Timely intervention of risk factors should be strengthened in clinical work,which is of great significance to reduce the fear emotion.
