盐酸曲唑酮联合正念练习治疗苯二氮[艹卓]类药物依赖慢性失眠的疗效观察

摘　　要：目的观察盐酸曲唑酮联合正念练习治疗苯二氮[艹卓]类药物依赖慢性失眠的疗效。方法苯二氮[艹卓]类药物依赖慢性失眠患者随机分为研究组和对照组,研究组逐渐减少苯二氮[艹卓]类药物剂量同时联合盐酸曲唑酮及正念练习进行治疗,对照组逐渐减少苯二氮[艹卓]类药物剂量同时只联合盐酸曲唑酮进行治疗,两组治疗前及治疗1月、3月、6月后进行匹兹堡睡眠质量指数量表(PSQI)、焦虑自评量表(SAS)、汉密尔顿焦虑量表(HAMA)、生活满意度量表(LSR)、抗抑郁药物副反应量表(SERS)及临床疗效评定。结果治疗6个月后研究组有效率70.00%,对照组有效率55.00%,差异显著(P<0.01);两组治疗1、3、6月后PSQI评分均低于治疗前,LSR评分均高于治疗前(P<0.05)。研究组3、6月后PSQI评分低于对照组,LSR评分高于对照组(P<0.05)。两组1、3、6月后SAS、HAMA评分低于治疗前(P<0.05)。研究组3月后HAMA评分低于对照组,6月后SAS、HAMA评分均低于对照组(P<0.05)。治疗后两组SERS评分组内、组间均无统计学意义(P>0.05)。结论盐酸曲唑酮联合正念治疗苯二氮[艹卓]类依赖慢性失眠患者可以有效改善患者对苯二氮[艹卓]类药物依赖,同时缓解患者焦虑情绪,改善睡眠质量,提高生活满意度。Objective To observe the efficacy of trazodone hydrochloride combined with mindfulness exercises in the treatment of benzodiazepine dependent chronic insomnia.Methods The chronic insomnia patients with benzodiazepines dependence were randomly divided into the study group and the control group.The study group gradually reduced the benzodiazepines dosage and simultaneously combined with trazodone hydrochloride and mindfulness exercises for treatment.The control group gradually reduced the benzodiazepines dosage and only combined with trazodone hydrochloride for treatment.The Pittsburgh sleep quality index(PSQI),self rating anxiety scale(SAS),Hamilton anxiety scale(HAMA),life satisfaction rating scale(LSR),antidepressant side effects scale(SERS)and clinical efficacy were evaluated before treatment and after one month,three months and six months of treatment in both groups.Results After 6 months of treatment,the study group had an effective rate of 70.00%,while the control group had an effective rate of 55.00%,significant difference(P<0.01).After 1,3,and 6 months,the PSQI scores of both groups were lower than before treatment,and the LSR scores were higher than before treatment(P<0.05).After 3 and 6 months,the PSQI score of the research group was lower than that of the control group,and the LSR score was higher than that of the control group(P<0.05).After 1,3,and 6 months,the SAS and HAMA scores of both groups were lower than before treatment(P<0.05).After 3 months,the HAMA score of the research group was lower than that of the control group,and after 6 months,both SAS and HAMA scores were lower than those of the control group(P<0.05).After treatment,there was no statistically significant difference in SERS scores within or between the two groups(P>0.05).Conclusion Trazodone hydrochloride combined with mindfulness can effectively improve the dependence of patients on benzodiazepines,relieve their anxiety,improve their sleep quality and improve their life satisfaction.
