摘 要:目的探究三维斑点追踪成像(3D-STI)联合Tei指数对甲状腺功能亢进(甲亢)患者左心功能的评价作用。方法选取甲亢患者60例(研究组)及同期健康体检者60例(对照组)作为研究对象。应用常规超声及3D-STI获得左心功能相关参数,比较两组各参数差异,并分析3D-STI参数联合Tei指数对甲亢患者左心功能的评估价值。结果两组性别、年龄、体重指数、心率、收缩压、舒张压、左心室射血分数(LVEF)、舒张末期左心室后壁厚度(LVPWT)、舒张末期室间隔厚度(IVST)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)相比,差异无统计学意义(P>0.05);研究组左心室整体面积应变(GAS)、左心室整体圆周应变(GCS)、左心室整体纵向应变(GLS)及Tei指数比对照组高,左心室整体径向应变(GRS)比对照组低(P<0.05);GAS、GRS、GCS、GLS及Tei指数单独及联合预测甲亢患者左心功能的曲线下面积(AUC)分别为0.694、0.719、0.752、0.736、0.805、0.887,联合预测的AUC最高(P<0.05)。结论3D-STI联合Tei指数可发现甲亢患者左心功能早期改变,联合应用可进一步提升评估效能,及早发现心功能异常,为临床诊疗提供依据。Objective To investigate the impact of three-dimensional speckle tracking imaging(3D-STI)+Tei index in the evaluation of left heart function in patients with hyperthyroidism.Methods Sixty patients with hyperthyroidism(study group)and 60 healthy subjects(control group)were selected as the study objects.Left heart function parameters were got by conventional ultrasound and 3D-STI.The difference of parameters between the two groups was compared,and the evaluation value of 3D-STI parameters combined with Tei index on left heart function in hyperthyroidism patients was analyzed.Results The differences in gender,age,body mass index,heart rate,systolic blood pressure,diastolic blood pressure,left ventricular ejection fraction(LVEF),left ventricular posterior wall thickness(LVPWT),interventricular septal thickness(IVST),left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)between the two groups showed non-statistical significance(P>0.05);Global area strain(GAS),global circum strain(GCS),global longitudinal strain(GLS)and Tei indexes in the study group were higher than those in the control group,but global radial strain(GRS)was lower(P<0.05);The area under the curve(AUC)of GAS,GRS,GCS,GLS and Tei indexes alone and in combination in prediction of left heart function in patients with hyperthyroidism were 0.694,0.719,0.752,0.736,0.805 and 0.887,respectively,and the AUC of combined prediction was the highest(P<0.05).Conclusions 3D-STI combined with Tei index can detect early changes in left heart function in patients with hyperthyroidism,and the combined application can further improve the evaluation efficiency,early detection of cardiac function abnormalities,and provide basis for clinical diagnosis and treatment.
