摘  要:目的研究产前超声对胎儿脐动脉闭塞妊娠结局的预测价值。方法收集分娩证实的胎儿脐动脉闭塞病例,脐动脉、大脑中动脉及静脉导管血流参数,羊水量,脐带螺旋指数,妊娠结局。结果中晚孕确诊脐动脉闭塞胎儿共25例,其中单胎妊娠24例,双胎之一1例。死胎7例,活产儿18例。死胎组中胎儿生长受限,脐动脉、大脑中动脉、静脉导管血流频谱异常及羊水过少发生率高于活产组。结论胎儿血流参数(指标)异常,尤其大脑中动脉和静脉导管异常,胎儿生长受限,羊水过少增加了宫内窘迫、胎死宫内或新生儿重症等不良妊娠结局的风险。Objective To study the predictive value of prenatal ultrasound on the pregnancy outcome of fetal umbilical artery occlusion.Methods Cases of fetal umbilical artery occlusion confirmed after delivery,blood flow parameters of umbilical artery,middle cerebral artery,and ductus venosus,amniotic fluid volume,umbilical coiling index,and pregnancy outcomes were retrospectively analyzed.Results There were a total of 25 cases of umbilical artery occlusion in fetuses,including 24 cases of singleton pregnancy and 1 case of twin pregnancy.Among them,there were 7 stillbirths and 18 live births.The incidence of fetal growth restriction(FGR),abnormal blood flow spectra in umbilical artery,middle cerebral artery and ductus venosus,and oligohydramnios was higher in the stillbirth group than in the live birth group.Conclusions Abnormal fetal blood flow parameters(indicators),especially abnormalities in middle cerebral artery and ductus venosus,FGR,and oligohydramnios,increase the risk of adverse pregnancy outcomes such as fetal distress,intrauterine fetal demise,or severe neonatal conditions.
