残角子宫妊娠的超声诊断及孕期监测

摘　　要：目的探讨超声在残角子宫妊娠诊断及孕期监测中的应用价值。方法回顾性分析11例可疑残角子宫妊娠患者临床表现、超声诊断、孕期监测及妊娠结局情况,探讨不同孕期各型残角子宫妊娠的超声特征。结果11例可疑残角子宫妊娠患者中交通型(Ⅰ型)2例,非交通型(Ⅱ型)7例,误诊病例2例;9例确诊病例中5例于早孕期行腹腔镜下残角子宫切除术,2例中孕期行经腹部残角子宫切除术,2例动态观察至孕晚期行剖宫产及残角子宫切除术、娩出活胎;残角子宫妊娠超声征象为妊娠侧宫体与宫颈、宫颈管间不存在延续性;妊娠侧宫体与对侧宫体间可存在或不存在连接;妊娠侧宫体肌层厚度、张力情况及胎盘与肌层关系为妊娠结局提供参考。结论残角子宫妊娠的超声征象对其诊断及孕期监测具有较好的价值。Objective To explore the application value of ultrasound in the diagnosis and monitoring of rudimentary horn pregnancy.Methods Clinical and ultrasonic data and pregnancy outcomes were analyzed retrospectively in 11 patients with suspicious rudimentary horn pregnancy,and the ultrasonic manifestations of different types of rudimentary horn pregnancy were summarized.Results Among 11 patients with suspicious rudimentary horn pregnancy,2 were typeⅠ,7 were typeⅡand 2 misdiagnosed cases;5 underwent laparoscopic surgery in the first trimester,2 cases underwent transabdominal hysterectomy of residual horn during the second trimester,and two live fetuses were delivered by cesarean section and hysterectomy of residual horn in the third trimester;The ultrasonic manifestations of rudimentary horn pregnancy were the absence of continuity between the uterine of pregnancy side and the cervix or cervical canal,and the rudimentary horn might or might not be connected with the normal cavity;The thickness,tension of the uterine body of pregnancy,and the placenta,posterior placental space provided reference for pregnancy outcome.Conclusions Ultrasonic manifestations of rudimentary horn pregnancy have a good value for diagnosis and monitoring.
