摘  要:目的探讨前列腺增生术后尿源性感染与细胞色素P450酶1A2(CYP1A2)基因多态性的关系及转化生长因子-β1(TGF-β1)/Smads信号通路指标的预测价值。方法选取青岛市城阳区人民医院2020年12月-2023年12月收治的178例前列腺增生术后患者,31例尿源性感染患者为感染组,147例无尿源性感染患者为非感染组,分析感染组病原菌分布,比较两组患者CYP1A2基因rs2069514-3859位点多态性,比较血清TGF-β1、Smad2、Smad3表达及三指标单独及联合检测对术后尿源性感染的诊断价值。结果感染组患者中段尿液标本培养出48株病原菌,革兰阴性菌、革兰阳性菌占比分别为60.42%、39.58%;与非感染组相比,感染组CYP1A2基因rs2069514-3859位点基因分型AA、等位基因A分布频率升高(P<0.05),等位基因G分布频率均降低(P<0.05);感染组血清TGF-β1、Smad2、Smad3水平与非感染组比较更高(P<0.05);与血清TGF-β1、Smad2和Smad3单独检测诊断曲线下面积(AUC)相比,联合检测诊断术后尿源性感染的AUC值为0.947,诊断价值更高(P<0.05),联合检测的特异度为97.96%,敏感度为83.87%。结论前列腺增生术后尿源性感染病原菌主要为革兰阴性菌,术后感染与CYP1A2基因多态性有关,且此类患者血清TGF-β1/Smads信号通路活化,TGF-β1、Smad2、Smad3联合检测对前列腺增生术后尿源性感染的诊断价值更高。OBJECTIVE To explore the relationship between postoperative urinary infection in the benign prostatic hyperplasia patients and cytochrome P450 enzyme 1A2(CYP1A2)gene polymorphism and analyze the predictive value of transforming growth factor-β1(TGF-β1)/Smads signaling pathways.METHODS A total of 178 patients with benign prostatic hyperplasia who received surgical procedures in Qingdao Chengyang People's Hospital from Dec.2020 to Dec.2023 were enrolled in the study,31 of whom had urinary infection and were assigned as the infection group,and 147 did not have urinary infection and were assigned as the non-infection group.The distribution of pathogens isolated from the infection group was analyzed.The polymorphisms at rs2069514-3859 locus of CYP1A2 gene were compared between the two groups of patients,the expression levels of serum TGF-βl,Smad2 and Smad3 were compared between the two groups of patients,and the values of single and joint detection of the three indexes in diagnosis of the postoperative urinary infection were analyzed.RESULTS Totally 48 strains of pathogens were isolated from midstream urine specimens of the patients in the infection group,60.42%of which were gram-negative bacteria,and 39.58%were gram-positive bacteria.The frequencies of genotype AA and allele A at rs2069514-3859 locus of CYP1A2 gene were higher in the infection group than in the non-infection group(P<0.05),while the frequency of allele G of the infection group was lower than that of the non-infection group(P<0.05).The levels of serum TGF-β1,Smad2 and Smad3 of the infection group were higher than those of the non-infection group(P<0.05).The area under the curve(AUC)of the joint detection of serum TGF-βl,Smad2 and Smad3 was 0.947,the diagnostic value was higher(P<0.05),and the specificity of the joint detection was 97.96%,with the sensitivity 83.87%.CONCLUSIONS The gram-negative bacteria are dominant among the pathogens isolated from the benign prostatic hyperplasia patients with postoperative urinary infection.The postoperative in
