深圳市医疗机构基于区域化感控平台的病原学送检管理实践及其效果

摘　　要：目的通过推进区域化医院感染管理平台质量建设,提升病原学送检率及管理效能。方法通过对过程监测数据进行提取,分析抗菌药物用药目的、执行时间缺失等问题。在点对点干预指导后,分析以上缺失率的改善程度,及其对病原学送检率等指标的提升效果。结果通过干预,全市抗菌药物用药目的缺失率由3.10%下降至2.97%(χ^(2)=55.062,P<0.001),执行时间缺失率由89.07%下降至53.50%(χ^(2)=587653.135,P<0.001)。全市抗菌药物治疗前病原学送检率由35.37%提升至49.80%(χ^(2)=20643.909,P<0.001),细菌培养送检率由23.54%提升至33.25%(χ^(2)=11114.465,P<0.001)。除一级医院外,其余医疗机构执行时间缺失率与病原学送检率均呈现负相关(P<0.001)。结论抗菌药物用药目的、执行时间缺失是影响病原学送检信息化监测质量的关键因素。“数据导航,聚焦问题,精准指导”改善活动有助于提升区域化医院感染监测平台对病原学送检工作的管理效能。OBJECTIVE To raise the etiological submission rate and improve the management efficiency by pushing forward the construction of quality of regional health care-associated infections management platform.METHODS The monitoring data of process were collected,the problems such as the purposes of use of antibiotics and missing rate of execution time were analyzed.The effects of the point-to-point intervention measures on the extent of raising the missing rate and etiological submission rate were observed.RESULTS The missing rate of purposes of use of antibiotics was decreased from 3.10%to 2.97%in the whole city after the intervention(χ^(2)=55.062,P<0.001),and the missing rate of execution time was decreased from 89.07%to 53.50%(χ^(2)=587653.135,P<0.001).The etiological submission rate of the whole city was increased from 35.37%to 49.80%before the antimicrobial therapy(χ^(2)=20643.909,P<0.001),and the submission rate of bacterial culture was increased from 23.54%to 33.25%(χ^(2)=11114.465,P<0.001).The missing rate of execution time was negatively correlated with the etiological submission rate in all of the medical institutions except the first grade hospitals(P<o.ooi).CONCLUSIONS The purpose of use of antibiotics and absence of execution time are the key factors for influencing the quality of informatization monitoring of etiological submission.The improvement activity'data navigation,focusing on problems,accurate guidance'may facilitate the improvement of management efficiency of etiological submission based on the regional infection prevention and control platform.
