摘 要:目的了解山东省二级及以上医疗机构医院感染监测系统应用现况。方法2024年1月10日—1月31日山东省医院感染管理质量控制中心采用线上问卷的方式对山东省16个地市的医疗机构进行调查。结果本次参与调查的278所医疗机构以公立、综合医院为主,感控专职人员床位比约为1:174,71.22%的医疗机构医院感染专职人员配备达标,不同类别、级别和等级的医疗机构之间达标率差异有统计学意义(P<0.05)。医院信息系统(HIS)、实验室信息系统(LIS)、电子病历系统(EMRS)、医院感染监测信息系统、手术麻醉系统、微生物检验系统模块和移动护理系统使用率分别是96.04%、91.01%、91.01%、92.45%、76.26%、66.19%和63.67%。2013—2020年医疗机构医院感染信息化监测软件上线增长较快。96.11%的医疗机构每日使用监测系统,57.20%的医疗机构至少每月会出现系统故障,52.14%的医疗机构需催促工程师解决故障。在综合性监测、现患率调查等13种监测模块中,新生儿重症监护病房(NICU)医院感染监测及手卫生依从率监测模块上线率为56.03%及75.88%,其他模块上线率均在80%以上。模块功能配置方面存在灵活性不足、功能不完善、不同系统间互联互通不畅等问题。结论山东省二级及以上医疗机构需要进一步加强医院感染专职人员队伍建设,提高医院感染监测系统信息化水平,降低监测软件故障频率,完善监测功能。OBJECTIVE To understand the current status of application of nosocomial infection surveillance system in secondary or above medical institutions of Shandong Province.METHODS From Jan.10,2024 to Jan.31,2024,an online questionnaire survey was conducted for the medical institutions of 16 prefectural and municipal area in Shandong Province by Shandong Province Nosocomial Infection Management Quality Control Center.RESULTS Among 278 medical institutions involved in the survey,public hospitals and general hospitals were dominant.The ratio of infection control professionals to the bed was about 1∶174.The allocation of the infection control professionals reached the standard in 71.22%of the medical institutions,and there was significant difference in the rate of reaching the standard among the different types,grades and scales of medical institutions(P<0.05).The utilization rates of hospital information system(HIS),laboratory information system(LIS),electronic medical records system(EMRS),nosocomial infection surveillance information system,surgical anesthesia system,microbiological testing modules and mobile nursing system were 96.04%,91.01%,91.01%,92.45%,76.26%,66.19%and 63.67%,respectively.The launching rate of the nosocomial infection information surveillance software witnessed a rapid growth in the medical institutions between 2013 and 2020.96.11%of the medical institutions used the surveillance system every day,57.20%of the medical institutions had the system breakdowns at least every month,and 52.14%of the medical institutions needed to urge engineers for solution.Among 13 types of surveillance modules such as comprehensive surveillance and prevalence rate survey,the launching rates of nosocomial infection surveillance module for neonatal intensive care unit(NICU)and hand hygiene compliance rate surveillance module were 56.03%and 75.88%,respectively;the launching rates of the other modules were more than 80%.There were such problems as limited flexibility,incomplete function and unsmooth poor interoperability be
