摘  要:目的分析川崎病合并肺炎支原体(MP)感染患儿核因子-κB(NF-κB)相关信号通路指标及T淋巴细胞亚群水平,为川崎病合并肺炎支原体感染的防治提供参考和依据。方法选取2021年12月—2023年12月扬州大学附属医院收治的153例川崎病患儿为研究对象,其中肺炎支原体感染组67例、非肺炎支原体感染组86例,分析患儿临床特征、NF-κB相关信号通路指标[κB激酶β(IKKβ)、NFκB抑制蛋白(IκBα)、NF-κB/p50和NF-κB/p65]及T淋巴细胞亚群水平,采用Pearson相关系数分析川崎病合并肺炎支原体感染患儿NF-κB相关信号通路指标与T淋巴细胞亚群水平的相关性。结果肺炎支原体感染组与非肺炎支原体感染组患儿发热持续时间、存在呼吸系统症状的患儿占比及NF-κB相关信号通路指标与T淋巴细胞亚群水平比较差异有统计学意义(P<0.05)。肺炎支原体感染组患儿发热持续时间(7.96±0.56)d较长,存在呼吸系统症状的患儿占比(76.12%)较高;NF-κB相关信号通路指标水平升高;外周血调节性T细胞(Treg)含量(2.07±0.29)×10^(7)/L、Treg/Th17(0.72±0.09)、CD_(4)^(+)/CD_(8)^(+)(2.05±0.12)水平较高。川崎病合并肺炎支原体感染患儿血清IKKβ、IκBα、NF-κB/p50及NF-κB/p65水平与外周血Treg细胞含量、Treg/Th17、CD_(4)^(+)/CD_(8)^(+)值呈正相关(P<0.05),与外周血Th17细胞含量、CD_(8)^(+)水平呈负相关(P<0.05)。结论川崎病合并肺炎支原体感染患儿多伴有持续发热及呼吸系统症状,可引起NF-κB相关信号通路指标及T淋巴细胞亚群水平异常表达。OBJECTIVE To observe the levels of nuclear factor-κB(NF-κB)-related signaling pathways and T lymphocyte subsets of the Kawasaki disease children complicated with Mycoplasma pneumoniae infection so as to provide guidance for prevention and treatment of the Kawasaki disease children complicated with M.pneumoniae infection.METHODS A total of 153 children with Kawasaki disease who were treated in the Affiliated Hospital of Yangzhou University from Dec.2021 to Dec.2023 were recruited as the research subjects and were divided into the M.pneumoniae(MP)infection group with 67 cases and the non-MP infection group with 86 cases.The clinical characteristics and the levels of NF-κB-related signaling pathway indexes[iκB kinaseβ(IKKβ),inhibitor of NF-κB(IκBα),NF-κB/p50 and NF-κB/p65]and T lymphocyte subsets were observed.The association between the levels of NF-κB-related signaling pathway indexes and the levels of T lymphocyte subsets of the Kawasaki disease children complicated with M.pneumoniae infection was analyzed by Pearson correlation coefficient.RESULTS There were significant differences in the fever duration,the proportion of children with respiratory system symptoms and the levels of NF-κB-related signaling pathway indexes and T lymphocyte subsets between the MP infection group and the non-MP infection group(P<0.05).The fever duration of the MP infection group was(7.96±0.56)days,longer than that of the non-MP infection group.The proportion of children with respiratory system symptoms was 76.12%in the MP infection group,higher than that in the non-MP infection group.The levels of NF-κB-related signaling pathway indexes of the MP infection group were higher than those of the non-MP infection group.The levels of peripheral blood regulatory T cell(Treg),Treg/Th17 and CD_(4)^(+)/CD_(8)^(+)of the MP infection group were respectively(2.07±0.29)×10^(7)/L,(0.72±0.09)and(2.05±0.12),higher than those of the non-MP infection group.The levels of serum IKKβ,IκBα,NF-κB/p50 and NF-κB/p65 of the Kawasaki dise
