摘  要:目的 探讨纳米银敷料联合重组人表皮生长因子(rhEGF)治疗Ⅱ度烧伤的有效性和安全性。方法 随机选取2021年1月至2022年1月收治的深部Ⅱ度烧伤患者60例,按照数字表法随机分成常规组和研究组,每组30例。常规组给予rhEGF治疗,研究组在常规组治疗的基础上联合纳米银敷料治疗,比较2组治疗后1、2、4周的创面愈合情况、细菌阳性检出率、完全溶痂时间、创面愈合时间、创面愈合后瘢痕增生程度,现象2组干预前后的疼痛程度[视觉模拟评分(VAS)]变化,比较2组不良反应发生情况。结果 治疗后1、2、4周时,研究组创面愈合率均高于常规组,差异有统计学意义(P<0.05)。研究组的细菌阳性检出率、完全溶痂时间、创面愈合时间和增生性瘢痕率均低于常规组,研究组的扁平瘢痕率高于常规组,差异有统计学意义(P<0.05)。治疗前,2组患者VAS评分比较,差异无统计学意义(P>0.05);治疗后,2组患者VAS评分均低于治疗前,且研究组低于常规组,差异有统计学意义(P<0.05)。2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论 纳米银敷料和rhEGF联合使用可显著促进烧伤患者的创面愈合,疗效好且安全性高,值得临床推广应用。Objective To explore the efficacy and safety of nano-silver dressing combined with recombinant human epidermal growth factor(rhEGF)on the treatment of deep second-degree burns.Methods A total of 60 patients with deep second-degree burns admitted from January 2021 to January 2022 were randomly divided into two groups according to the digital table method.The conventional group was treated with rhEGF,and the study group was treated with nano-silver dressing combined with rhEGF.The wound healing,positive bacterial detection rate,complete dissolution scab time,wound healing time,and degree of scar hyperplasia after wound healing at weeks 1,2,and 4 after treatment,the visual analogue scale(VAS)for pain,the incidence of adverse events were compared between the two groups.Results At weeks 1,2,and 4 after treatment,the wound healing rates in the study group were significantly higher than those in the conventional group(P<0.05).The positive bacterial detection rate,complete dissolution scab time,wound healing time,and hypertrophic scar rate in the study group were significantly lower than those in the conventional group,and the flat scar rate was significantly higher than that in the conventional group(P<0.05).After treatment,the VAS scores of the two groups were significantly lower than those before treatment,which were significantly lower in the study group than the conventional group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion The combined use of nano-silver dressing and rhEGF can significantly promote wound healing of burn patients with good efficacy and high safety,which is worthy of clinical promotion.
