单硝酸异山梨酯联合培哚普利叔丁胺治疗对慢性心力衰竭患者心功能及炎性指标的影响

摘　　要：目的探讨单硝酸异山梨酯联合培哚普利叔丁胺治疗对慢性心力衰竭(CHF)患者心功能及炎性指标的影响。方法选取2020年10月—2023年11月山东省第一荣军优抚医院收治的76例CHF患者为研究对象,按随机数字表法将其分为对照组和观察组,各38例。对照组采用培哚普利叔丁胺治疗,观察组采用单硝酸异山梨酯联合培哚普利叔丁胺治疗。对比两组的心功能指标、炎性指标、心力衰竭标志物水平及不良反应发生情况。结果治疗后,两组左心室射血分数(LVEF)均高于治疗前,且观察组LVEF高于对照组,两组左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)均短于治疗前,且观察组LVEDD、LVESD均短于对照组,差异有统计学意义(P<0.05);两组肿瘤坏死因子-α、C反应蛋白、白细胞介素-6水平均低于治疗前,且观察组均低于对照组,差异有统计学意义(P<0.05);两组心肌肌钙蛋白T(cTnT)、N末端脑钠肽前体(NT-proBNP)水平均低于治疗前,且观察组cTnT、NT-proBNP水平均低于对照组,两组胰岛素生长因子-1(IGF-1)水平均高于治疗前,且观察组IGF-1水平高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论单硝酸异山梨酯联合培哚普利叔丁胺治疗CHF患者的效果更佳,能有效改善患者心功能指标、炎性指标及心力衰竭标志物水平,且安全可靠。Objective Exploring the effects of isosorbide mononitrate combined with perindopril tert butylamine on cardiac function and inflammatory indicators in patients with chronic heart failure(CHF).Methods Selecting 76 CHF patients admitted to The First Rongjun Special Care Hospital in Shandong Province from October 2020 to November 2023 as the research subjects,according to the random number table method,they were divided into a control group and an observation group,with 38 cases in each group.The control group was treated with perindopril tert butylamine,while the observation group was treated with isosorbide mononitrate combined with perindopril tert butylamine.Compare the cardiac function indicators,inflammatory indicators,heart failure biomarkers levels,and occurrence of adverse reactions between two groups.Results After treatment,the left ventricular ejection fraction(LVEF)in both groups was higher than before treatment,and the LVEF in the observation group was higher than that in the control group,the left ventricular end diastolic diameter(LVEDD)and left ventricular end systolic diameter(LVESD)in both groups were shorter than before treatment,and the LVEDD and LVESD in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05);the levels of tumor necrosis factor-α,C-reactive protein,and interleukin-6 in both groups were lower than before treatment,and the observation group was lower than the control group,the differences were statistically significant(P<0.05);the levels of cardiac troponin T(cTnT)and N-terminal pro-brain natriuretic peptide(NT-proBNP)in both groups were lower than before treatment,and the average levels of cTnT and NT-proBNP in the observation group were lower than those in the control group,the levels of insulin growth factor-1(IGF-1)in both groups were higher than before treatment,and the levels of IGF-1 in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Co
