血清IL-6联合hs-CRP对慢性阻塞性肺疾病急性加重期患者短期预后预测价值分析

摘　　要：目的分析血清白细胞介素-6(IL-6)联合超敏C反应蛋白(hs-CRP)对慢性阻塞性肺疾病急性加重期(AECOPD)患者短期预后的预测价值。方法回顾性分析2022年1月—2022年12月徐州市铜山区中医院收治的128例AECOPD患者的病历资料,根据预后将其分为预后良好组和预后不良组。采用Logistic回归分析AECOPD患者短期预后的影响因素;采用受试者工作特征曲线分析血清IL-6联合hs-CRP对AECOPD患者短期不良的预测价值。结果单因素、Logistic回归分析显示,年龄、吸烟史、合并高血压、血清IL-6、hs-CRP是AECOPD患者预后不良的危险因素(P<0.05);ROC曲线显示,血清IL-6联合hs-CRP对AECOPD患者短期预后的曲线下面积为0.902,高于单独预测。结论年龄、吸烟史、合并高血压、血清IL-6、hs-CRP是AECOPD患者预后不良的危险因素,血清IL-6联合hs-CRP对AECOPD患者短期预后有较高预测效能。Objective To analyze the predictive value of serum interleukin-6(IL-6)combined with hypersensitive C-reactive protein(hs-CRP)in short-term prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods The medical records of 128 patients with AECOPD admitted to Tongshan District Hospital of Traditional Chinese Medicine of Xuzhou City from January 2022 to December 2022 were retrospectively analyzed,and divided into a good prognosis group and a poor prognosis group according to the prognosis.Logistic regression was used to analyze the factors affecting the short-term prognosis of AECOPD patients.The predictive value of serum IL-6 combined with hs-CRP for short-term adverse effects in patients with AECOPD was analyzed by subject operating characteristic curve.Results Univariate and Logistic regression analysis showed that age,smoking history,combined hypertension,serum IL-6 and hs-CRP were the risk factors for poor prognosis in AECOPD patients(P<0.05).ROC curve showed that the area under the curve of serum IL-6 combined with hs-CRP on the short-term prognosis of AECOPD patients was 0.902,which was higher than that predicted by alone.Conclusion Age,smoking history,combined hypertension,serum IL-6 and hs-CRP are risk factors for poor prognosis of AECOPD patients,serum IL-6 combined with hs-CRP has a high predictive effect on short-term prognosis of AECOPD patients.
