闭合复位弹性髓内钉固定术联合外固定架对肱骨近端骨折患儿手术相关指标及肩关节功能的影响

摘　　要：目的探讨闭合复位弹性髓内钉固定术联合外固定架对肱骨近端骨折患儿手术相关指标及肩关节功能的影响。方法选择2021年6月—2023年7月莘县中医医院收治的82例肱骨近端骨折患儿为研究对象,按随机数字表法将其分为对照组和观察组,各41例。对照组行切开复位锁定钢板内固定术,观察组行闭合复位弹性髓内钉固定术联合外固定架,两组均观察至术后6个月。比较两组的手术相关指标、肩关节功能、并发症发生情况、生活质量。结果观察组手术时间、切口长度均短于对照组,术中出血量少于对照组,术后24 h疼痛视觉模拟评分低于对照组,组间差异有统计学意义(P<0.05)。术后6个月,观察组Constant-Murley肩关节功能评分量表各个维度评分均高于对照组,组间差异有统计学意义(P<0.05);观察组儿童生活质量普适性核心评定量表各个领域评分均高于对照组,组间差异有统计学意义(P<0.05)。观察组并发症发生率为4.88%,低于对照组的19.51%,差异有统计学意义(P<0.05)。结论与切开复位锁定钢板内固定术相比,闭合复位弹性髓内钉固定术联合外固定架治疗肱骨近端骨折患儿可进一步优化手术相关指标,改善肩关节功能和生活质量,且并发症少。Objective Exploring the effects of closed reduction elastic intramedullary nail fixation combined with external fixation on surgical related indicators and shoulder joint function in children with proximal humeral fractures.Methods A total of 82 children with proximal humeral fractures admitted to Shen County Traditional Chinese Medicine Hospital from June 2021 to July 2023 were selected as the research subjects,according to the random number table method,they were divided into a control group and an observation group,with 41 cases in each group.The control group underwent open reduction and locking steel plate internal fixation,while the observation group underwent closed reduction elastic intramedullary nail fixation combined with external fixation,both groups were observed until 6 months after surgery.Compare surgical related indicators,shoulder joint function,occurrence of complications,and quality of life between two groups.Results surgical time and incision length of the observation group were shorter than those of the control group,the intraoperative bleeding was less than that of the control group,and the postoperative 24 hours Visual Analog Score score of pain was lower than that of the control group,the differences between the groups were statistically significant(P<0.05).Six months after surgery,the scores in all dimensions of the Constant-Murley Shoulder Joint Function Rating Scale in the observation group were higher than those in the control group,the differences between the groups were statistically significant(P<0.05);the scores in all areas of the Pediatric Quality of Life Inventory Measurement Models Version 4 in the observation group were higher than those in the control group,the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 4.88%,which was lower than the 19.51%in the control group,the difference was statistically significant(P<0.05).Conclusion Compared with open reduction and locking steel plate internal fixat
