空心钉与微型钢板内固定手术对创伤性后踝大块骨折患者术后恢复及踝关节功能的影响比较

摘　　要：目的探讨空心钉与微型钢板内固定手术对创伤性后踝大块骨折患者术后恢复及踝关节功能的影响。方法选择2022年2月—2023年2月莒县人民医院收治的94例创伤性后踝大块骨折患者为研究对象,按随机数字表法将其分为对照组和观察组,各47例。对照组行空心钉内固定手术治疗,观察组行微型钢板内固定手术治疗。比较两组的术后恢复情况、踝关节功能及炎性因子水平。结果观察组术后住院时间、下床活动时间、骨折愈合时间均短于对照组,组间差异有统计学意义(P<0.05)。术后6个月,两组Olerud-Molander踝关节评分、美国足踝外科协会踝关节功能评分均高于术前,且观察组均高于对照组,差异有统计学意义(P<0.05)。术后1个月,两组白细胞介素-8、白细胞介素-6、C反应蛋白水平均低于术前,且观察组均低于对照组,差异有统计学意义(P<0.05)。结论微型钢板内固定手术治疗创伤性后踝大块骨折患者可加快术后恢复速度,改善踝关节功能,降低炎性因子水平。Objective Exploring the effects of hollow nail and mini steel plate internal fixation surgery on postoperative recovery and ankle joint function in patients with traumatic large ankle fractures.Methods A total of 94 patients with traumatic large ankle fractures admitted to Ju County People's Hospital from February 2022 to February 2023 were selected as the research subjects,and were divided into a control group and an observation group according to random number table method,with 47 cases in each group.The control group underwent hollow nail internal fixation surgery,while the observation group underwent mini steel plate internal fixation surgery.Compare the postoperative recovery status,ankle joint function,and inflammatory factor levels between two groups.Results The postoperative hospitalization time,mobilization time,and fracture healing time of the observation group were shorter than those of the control group,the differences between the groups were statistically significant(P<0.05).Six months after surgery,the Olerud-Molander Ankle joint score and American Orthopaedic Foot and Ankle Society ankle joint function score in both groups were higher than those before surgery,and the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).One month after surgery,the levels of interleukin-8,interleukin-6,and C-reactive protein in both groups were lower than those before surgery,and the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion Mini steel plate internal fixation surgery can accelerate postoperative recovery speed,improve ankle joint function,and reduce inflammatory factor levels in patients with traumatic large ankle fractures.
