摘 要:目的探讨超声引导下针刀联合神经阻滞治疗轻中度腕管综合征的临床疗效。方法择取轻中度腕管综合征患者41例为研究对象,经随机抽签法分为实验组(21例,超声引导下针刀联合神经阻滞治疗)、对照组(20例,传统针刀联合神经阻滞治疗)。比较两组治疗效果,治疗前后腕管功能、疼痛程度评分、电生理参数、超声指标。结果实验组治疗优良率100.00%高于对照组的75.00%(P<0.05)。治疗1、3个月后,两组腕管功能、疼痛程度评分均明显低于本组治疗前,且实验组腕管功能评分(25.01±2.49)、(21.10±1.42)分和疼痛程度评分(2.17±0.42)、(0.81±0.24)分均明显低于对照组的(29.34±2.50)、(25.55±1.46)分和(2.87±0.46)、(1.95±0.36)分(P<0.05)。治疗1、3个月后,两组神经末梢运动潜伏期(DML)、感觉传导速度(SCV)均明显优于本组治疗前,且实验组DML(4.10±0.26)、(3.70±0.23)ms低于对照组的(4.36±0.28)、(4.01±0.25)ms,SCV(44.87±2.26)、(47.75±2.56)m/s大于对照组的(42.41±2.20)、(45.10±2.48)m/s(P<0.05)。治疗1、3个月后,两组正中神经最肿胀处横截面积(CSA)、正中神经扁平率(FR)均明显优于本组治疗前,且实验组CSA(12.54±1.32)、(10.42±1.11)mm~2和FR(2.50±0.20)、(2.00±0.16)均小于对照组的(14.00±1.35)、(11.65±1.25)mm~2和(2.75±0.23)、(2.31±0.22)(P<0.05)。结论轻中度腕管综合征患者实施超声引导下针刀联合神经阻滞治疗,可提高临床疗效,改善腕管功能、疼痛程度、电生理指标、超声指标,应用价值显著。Objective To explore the clinical efficacy of ultrasound-guided needle-knife combined with nerve block for the treatment of mild-to-moderate carpal tunnel syndrome.Methods 41 patients with mild-to-moderate carpal tunnel syndrome were selected as study subjects,and were divided into experimental group(21 cases,ultrasound-guided needle-knife combined with nerve block)and control group(20 cases,traditional needle-knife combined with nerve block).The therapeutic effect,carpal tunnel function,pain score,electrophysiological parameters and ultrasonic indexes before and after treatment were compared between the two groups.Results The excellent rate of treatment in the experimental group was 100.00%,which was higher than 75.00%in the control group(P<0.05).After 1 and 3 months of treatment,the scores of carpal tunnel function and pain in both groups were significantly lower than those before treatment;the carpal tunnel function score in the experimental group were(25.01±2.49)and(21.10±1.42)points,and the pain scores were(2.17±0.42)and(0.81±0.24)points,which were significantly lower than(29.34±2.50),(25.55±1.46)points and(2.87±0.46),(1.95±0.36)points in the control group(P<0.05).After 1 and 3 months of treatment,the distal motor latency(DML)and sensory conduction velocity(SCV)in both groups were significantly better than those before treatment;DML were(4.10±0.26)and(3.70±0.23)ms in the experimental group,which were lower than(4.36±0.28)and(4.01±0.25)ms in the control group;SCV were(44.87±2.26)and(47.75±2.56)m/s in the experimental group,which were higher than(42.41±2.20)and(45.10±2.48)m/s in the control group(P<0.05).After 1 and 3 months of treatment,the cross-sectional area(CSA)and the flattening ratio(FR)in both groups were significantly better than those before treatment;CSA were(12.54±1.32)and(10.42±1.11)mm2 in the experimental group,and FR 2 were(2.50±0.20)and(2.00±0.16),which were smaller than(14.00±1.35),(11.65±1.25)mm and(2.75±0.23),(2.31±0.22)in the control group(P<0.05).Conclusion Ultrasou
