摘 要:目的探讨尿微量白蛋白(mAlb)、免疫球蛋白G(IgG)、α_(1)-微球蛋白(α_(1)-MG)、β_(2)-微球蛋白(β_(2)-MG)对糖尿病肾病(DKD)患者肾损伤程度的诊断价值。方法选择120例DKD患者作为DKD组,根据肾小球滤过率水平不同将DKD患者分为轻度损伤组(19例)、中度损伤组(81例)、重度损伤组(20例);另选120例单纯糖尿病患者作为糖尿病组,并选取同期健康体检者120例作为健康对照组。检测并对比三组尿mAlb、IgG、α_(1)-MG、β_(2)-MG水平;对比不同程度肾损伤DKD患者尿m Alb、IgG、α_(1)-MG、β_(2)-MG水平;对比DKD组和糖尿病组患者尿mAlb、IgG、α_(1)-MG、β_(2)-MG单独及联合检测的阳性检出率。结果DKD组尿mAlb、IgG、α_(1)-MG及β_(2)-MG水平均高于糖尿病组、健康对照组,糖尿病组尿mAlb、IgG、α_(1)-MG及β_(2)-MG水平均高于健康对照组,差异有统计学意义(P<0.05)。不同程度肾损伤DKD患者尿mAlb、IgG、α_(1)-MG、β_(2)-MG水平对比差异有统计学意义(P<0.05),DKD患者肾损伤程度越重,尿mAlb、IgG、α_(1)-MG、β_(2)-MG水平越高。重度损伤组尿mAlb、IgG、α_(1)-MG、β_(2)-MG水平均高于轻度损伤组、中度损伤组,且中度损伤组高于轻度损伤组,差异有统计学意义(P<0.05)。DKD组尿mAlb、IgG、α_(1)-MG、β_(2)-MG单独及联合检测的阳性检出率分别为77.50%、76.67%、80.00%、78.33%、97.50%,均较糖尿病组的1.67%、1.67%、3.33%、3.33%、10.00%高,差异有统计学意义(P<0.05);两组尿mAlb、IgG、α_(1)-MG、β_(2)-MG单独检测的阳性检出率均较联合检测低,差异有统计学意义(P<0.05)。结论尿mAlb、IgG、α_(1)-MG、β_(2)-MG在DKD患者中的阳性检出率高,不同程度肾损伤患者的水平也不同,可用于临床病情诊断和治疗中,诊断价值理想,值得应用。Objective To explore the diagnostic value of urinary microalbuminuria(mAlb),immunoglobulin G(IgG),a_(1)-microglobulin(a_(1)-MG)andβ_(2)-microglobulin(β_(2)-MG)on degree of renal injury in patients with diabetic kidney disease(DKD).Methods 120 patients with DKD were selected as DKD group.According to the different glomerular filtration rates,the DKD patients were divided into mild injury group(19 cases),moderate injury group(81 cases)and severe injury group(20 cases).Another 120 patients with simple diabetes were selected as diabetes group,and 120 health examinees in the same period were selected as healthy control group.The levels of urinary mAlb,IgG,a_(1)-MG,β_(2)-MG were compared among the three groups;the levels of urinary mAlb,IgG,a_(1)-MG,β_(2)-MG were compared in DKD patients with different degrees of renal injury;the positive rates of urinary mAlb,IgG,a_(1)-MG,β_(2)-MG alone and in combination were compared between DKD group and diabetes group.Results The levels of urinary mAlb,IgG,a_(1)-MG andβ_(2)-MG in DKD group were higher than those in diabetes group and healthy control group,and the levels of urinary mAlb,IgG,a_(1)-MG andβ_(2)-MG in diabetes group were higher than those in healthy control group.The difference was statistically significant(P<0.05).There were significant differences in levels of urinary mAlb,IgG,a_(1)-MG,β_(2)-MG in DKD patients with different degrees of renal injury(P<0.05).The more severe the renal injury in DKD patients,the higher the levels of urinary mAlb,IgG,a_(1)-MG,andβ_(2)-MG.The levels of urinary mAlb,IgG,a_(1)-MG andβ_(2)-MG in severe injury group were higher than those in mild injury group and moderate injury group,and the moderate injury group was higher than the mild injury group.The difference was statistically significant(P<0.05).The positive rates of urinary mAlb,IgG,a_(1)-MG andβ_(2)-MG alone and in combination were 77.50%,76.67%,80.00%,78.33%and 97.50%in DKD group,which were higher than 1.67%,1.67%,3.33%,3.33%and 10.00%in diabetes group.The difference was
