摘 要:目的评估三维水晶成像技术在胎儿脊柱产前超声检查中的应用价值。方法选取51例行产前超声检查的孕妇,均行二维超声及三维水晶成像技术,留图保存,由两位高年资医师进行分析其成像效果。结果51例孕妇,其中包括2例半椎体胎儿。49例孕妇采用三维水晶成像技术可在不同切面上直接评估脊柱的整个发育情况,其余2例由于声窗不佳(1例)及胎儿持续躺卧不佳(1例)等技术因素,整个胎儿脊柱的可视化效果并不理想。三维水晶成像显示2例存在脊柱畸形,因其未伴有脊柱侧弯,在二维超声图像中不易察觉;其中1例冠状切面面上胸12半椎体畸形,其余椎体排列整齐。引产后得到证实。三维水晶成像技术对椎体椎弓矢状切面显示良好率、脊柱冠状切面显示良好率、脊髓圆锥定位准确率分别为96%、96%、96%,二维超声分别为96%、0、82%,三维水晶成像技术对脊柱冠状切面显示良好率、脊髓圆锥定位准确率高于二维超声,差异显著(P<0.05)。结论三维水晶成像技术在胎儿脊柱检查中具有更好的显示效果,且耗时短,有助于产前咨询和处理疑似脊柱畸形胎儿。Objective To evaluate the difference between 3D-Crystal Vue and conventional 2D ultrasound in the display rate of fetal spine and other relevant factors,and discuss the advantages of 3D-Crystal Vue.Methods 51 pregnant women who underwent prenatal ultrasound were selected and underwent 2D ultrasound and 3D-Crystal Vue,the images were retained and saved,and the imaging results were analyzed by two senior physicians.Results There were 51 pregnant women,including 2 hemivertebral fetuses.Subjective evaluation of the imaging effect by two senior physicians showed that 3D-Crystal Vue provided clearer anatomical structures and details:the good display rate of sagittal section of vertebral arch,the good display rate of coronal section of vertebral column,and the accuracy rate of spinal conus positioning by 3D-Crystal Vue were 96%,96%,and 96%,while those by 2D ultrasound were 96%,0,and 82%;the good display rate of 3D-Crystal Vue was higher than that of 2D ultrasound,and the difference was significant(P<0.05).In 49 pregnant women,the entire development of the spine could be directly assessed in different sections by 3D-Crystal Vue.In the remaining 2 cases,visualization of the entire fetal spine was not satisfactory due to technical factors such as poor acoustic window(1 case)and poor sustained fetal lying(1 case).3D-Crystal Vue showed the presence of spinal deformity in 2 cases,which was not easily detected in 2D ultrasound images because it was not accompanied by scoliosis.1 case had a thoracic 12 hemivertebra deformity on coronal section,and the rest of the vertebrae were well aligned.It was confirmed after induction of labor.Conclusion 3D-Crystal Vue provides better visualization of the fetal spine on ultrasound and is less time-consuming,which will facilitate prenatal counseling and management of patients with suspected spinal deformities.
