阿卡波糖片联合甘精胰岛素治疗老年糖尿病患者的效果评价

摘　　要：目的分析老年糖尿病应用阿卡波糖片联合甘精胰岛素治疗的效果。方法70例老年糖尿病患者,运用奇偶数形式分为对照组和观察组,每组35例。对照组予以甘精胰岛素治疗,观察组予以阿卡波糖片联合甘精胰岛素治疗。对比两组患者血糖(空腹血糖、餐后2 h血糖、糖化血红蛋白)、胰岛素指标(胰岛β细胞功能指数、空腹胰岛素、餐后2 h胰岛素)、血脂代谢指标(总胆固醇、甘油三酯、载脂蛋白A)、氧化应激指标(谷胱甘肽过氧化物酶、脂质过氧化氢、总抗氧化能力)、不良反应发生情况、跟踪回访6个月的并发症发生情况。结果治疗后,观察组患者空腹血糖(6.03±0.29)mmol/L、餐后2 h血糖(7.23±1.12)mmol/L、糖化血红蛋白(7.44±1.32)%均低于对照组的(7.94±0.41)mmol/L、(9.36±1.48)mmol/L、(9.53±1.20)%,胰岛β细胞功能指数(34.52±3.27)、空腹胰岛素(15.69±1.20)μIU/ml、餐后2 h胰岛素(25.56±3.11)μIU/ml均高于对照组的(26.91±4.26)、(11.26±1.34)μIU/ml、(20.43±2.36)μIU/ml(P<0.05)。治疗后,观察组患者总胆固醇(2.80±0.25)mmol/L、甘油三酯(1.38±0.30)mmol/L、脂质过氧化氢(315.63±31.45)μmol/L均低于对照组的(4.18±0.36)mmol/L、(1.63±0.47)mmol/L、(421.17±35.29)μmol/L,载脂蛋白A(1.23±0.09)g/L、谷胱甘肽过氧化物酶(73.42±3.36)U/L、总抗氧化能力(20.75±3.48)U/ml均高于对照组的(1.04±0.10)g/L、(64.05±4.17)U/L、(12.17±4.06)U/ml(P<0.05)。观察组不良反应发生率2.86%、并发症发生率2.86%均低于对照组的17.14%、20.00%(P<0.05)。结论为老年糖尿病患者实施阿卡波糖片、甘精胰岛素联合治疗,可见良好降糖功效,且患者治疗后发症和不良反应得到有效管控,建议推广。Objective To analyze the effect of acarbose tablet combined with insulin glargine in the treatment of elderly patients with diabetes.Methods 70 elderly patients with diabetes were divided into control group and observation group by odd and even numbers,with 35 cases in each group.The control group was treated with insulin glargine,and the observation group was treated with acarbose tablets combined with insulin glargine.Patients in both groups were compared in terms of blood glucose(fasting blood glucose,2 h postprandial blood glucose,glycated hemoglobin),insulin indexes(isletβ-cell function index,fasting insulin,2 h postprandial insulin),lipid metabolism indexes(total cholesterol,triglycerides,apolipoprotein A),oxidative stress indexes(glutathione peroxidase,lipid hydroperoxide,total antioxidant capacity),occurrence of adverse reactions,and complications after 6 months of follow-up.Results After treatment,in the observation group,the fasting blood glucose was(6.03±0.29)mmol/L,2 h postprandial blood glucose was(7.23±1.12)mmol/L,and the glycated hemoglobin was(7.44±1.32)%,which were lower than(7.94±0.41)mmol/L,(9.36±1.48)mmol/L and(9.53±1.20)%in the control group;the isletβ-cell function index was(34.52±3.27),the fasting insulin was(15.69±1.20)μIU/ml,and the 2 h postprandial insulin was(25.56±3.11)μIU/ml,which were higher than(26.91±4.26),(11.26±1.34)μIU/ml,and(20.43±2.36)μIU/ml in the control group(P<0.05).After treatment,in the observation group,the total cholesterol was(2.80±0.25)mmol/L,the triglyceride was(1.38±0.30)mmol/L,and the lipid hydroperoxide was(315.63±31.45)μmol/L,which were lower than(4.18±0.36)mmol/L,(1.63±0.47)mmol/L,and(421.17±35.29)μmol/L in the control group;the apolipoprotein A was(1.23±0.09)g/L,the glutathione peroxidase was(73.42±3.36)U/L,and the total antioxidant capacity was(20.75±3.48)U/ml,which were higher than(1.04±0.10)g/L,(64.05±4.17)U/L,and(12.17±4.06)U/ml in the control group(P<0.05).The incidence of adverse reactions and complications in the observat
