基于“治未病”思想观察健脾泻浊汤防治脾虚湿滞型痛风性关节炎的临床疗效

摘　　要：目的探讨采用健脾泻浊汤治疗脾虚湿滞型痛风性关节炎的临床疗效。方法65例脾虚湿滞型痛风性关节炎患者,随机分为对照组(32例)与治疗组(33例)。对照组予以基础治疗,治疗组予以基础治疗联合健脾泻浊汤治疗。比较两组临床疗效及治疗前后实验室指标[C反应蛋白(CRP)、血尿酸(UA)]、中医证候积分、关节疼痛程度[视觉模拟评分法(VAS)评分]。结果治疗后,两组UA及CRP均较治疗前下降,且治疗组UA(325.24±18.26)μmol/L及CRP(12.32±1.82)mg/L较对照组的(382.33±18.45)μmol/L、(25.34±6.75)mg/L下降更明显,差异有统计学意义(P<0.05)。治疗后,两组VAS评分均较治疗前下降,且治疗组VAS评分(0.78±0.25)分较对照组的(3.12±1.55)分下降更明显,差异有统计学意义(P<0.05)。治疗后,两组中医证候积分均较治疗前下降,且治疗组中医证候积分(9.75±1.09)分较对照组的(18.77±2.98)分下降更明显,差异有统计学意义(P<0.05)。治疗组总有效率为97.0%,对照组总有效率为78.1%。治疗组总有效率高于对照组,差异有统计学意义(P<0.05)。结论基于“治未病”思想指导应用健脾泻浊汤治疗脾虚湿滞型痛风性关节炎的临床效果显著,可明显改善患者的UA水平,提高临床疗效,值得在临床上推广应用。Objective To explore the clinical efficacy of Jianpi Xiezhuo Decoction in the prevention and treatment of gouty arthritis of spleen deficiency and dampness stagnation type.Methods 65 patients with gouty arthritis of spleen deficiency and dampness stagnation type were divided into the control group(32 cases)and the treatment group(33 cases).The control group was only given basic treatment,and the treatment group was given basic treatment plus Jianpi Xiezhuo Decoction.Comparison was made on clinical efficacy and laboratory indicators[C-reactive protein(CRP),blood uric acid(UA)],traditional Chinese medicine syndrome score,and degree of joint pain[Visual Analog Scale(VAS)score]before and after treatment between the two groups.Results After treatment,UA and CRP in both groups decreased compared with those before treatment;the treatment group had UA of(325.24±18.26)μmol/L and CRP of(12.32±1.82)mg/L,which were significantly lower than(382.33±18.45)μmol/L and(25.34±6.75)mg/L in the control group,and the difference was statistically significant(P<0.05).After treatment,VAS score in both groups decreased compared with that before treatment,and VAS score of(0.78±0.25)points in the treatment group decreased more significantly than(3.12±1.55)points in the control group.The difference was statistically significant(P<0.05).After treatment,the traditional Chinese medicine syndrome score in both groups decreased compared with that before treatment,and the traditional Chinese medicine syndrome score of(9.75±1.09)points in the treatment group decreased more significantly than(18.77±2.98)points in the control group.The difference was statistically significant(P<0.05).The total effective rate was 97.0%in the treatment group and 78.1%in the control group;the total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Based on the idea of"preventing disease",the application of Jianpi Xiezhuo Decoction has significant clinical effect,w
