机采血小板献血者铁蛋白降低影响因素分析及其对血常规指标、铁蛋白的影响

摘　　要：目的分析机采血小板献血者铁蛋白降低影响因素及其对血常规指标、铁蛋白的影响。方法选取2022年9月—2023年8月珠海市中心血站235名连续机采血小板献血者作为研究对象,进行血常规各项指标和铁蛋白水平检测,分析机采血小板献血者铁蛋白降低的影响因素及其对血常规各项指标、铁蛋白的影响。结果单因素分析结果显示,235名机采血小板献血者中,铁蛋白降低率为22.55%(53/235),不同性别、总献血次数与献血频率献血者铁蛋白降低率比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,总献血次数和献血频率是机采血小板献血者铁蛋白降低的影响因素(P<0.05)。总献血次数越多机采血小板献血者的红细胞比容、铁蛋白水平越低,献血频率越高机采血小板献血者的铁蛋白水平越低,差异有统计学意义(P<0.05)。结论机采血小板献血者铁蛋白降低受总献血次数、献血频率的影响较大,建议采血机构对总献血次数≥50次和献血频率≥10次/年的机采血小板献血者增加单采前铁蛋白水平检测,对铁蛋白降低的机采血小板献血者采取口服铁剂补充和适当延长献血间隔等措施,确保献血者健康和血液安全。Objective To analyze the influencing factors of ferritin reduction in apheresis platelet donors and its effects on blood routine indexes and ferritin levels.Methods A total of 235 consecutive apheresis platelet donors in Zhuhai Central Blood Station from September 2022 to August 2023 were selected as the research objects.The blood routine indexes and ferritin levels were detected,and the influencing factors of ferritin reduction in apheresis platelet donors and its effects on blood routine indexes and ferritin levels were analyzed.Results The result of univariate analysis showed that the ferritin reduction rate in 235 apheresis platelet donors was 22.55%(53/235).There were statistically significant differences in the low ferritin reduction rate among blood donors of different genders,total blood donation times and blood donation frequency(P<0.05).The result of multivariate Logistic regression analysis showed that the total number of blood donations and the frequency of blood donation were the influencing factors of the ferritin reduction in apheresis platelet donors(P<0.05).Among apheresis platelet donors,the higher the total number of blood donations,the lower the hematocrit and ferritin levels;the higher the frequency of blood donation,the lower the ferritin level,and the differences were statistically significant(P<0.05).Conclusions The ferritin reduction rate in apheresis platelet donors is greatly affected by the total number of blood donations and the frequency of blood donation.It is recommended that blood collection institutions should increase the detection of pre-apheresis ferritin levels for apheresis platelet donors with a total number of blood donations equal or more than 50 times and a blood donation frequency equal or more than 10 times/year.Measures such as oral iron supplementation and appropriate extension of blood donation intervals should be taken to ensure the health and blood safety of blood donors for apheresis platelet donors with ferritin reduction.
