破格救心汤加减治疗伴广泛耐药或全耐药鲍曼不动杆菌感染重症肺炎阳脱证患者的临床效果

摘　　要：目的观察破格救心汤加减治疗伴广泛耐药鲍曼不动杆菌(XDRAB)或全耐药鲍曼不动杆菌(PDRAB)感染重症肺炎阳脱证患者的临床效果。方法选取2020年10月—2023年5月遵义市中医院收治的伴XDRAB或PDRAB感染的重症肺炎阳脱证患者48例作为研究对象,根据不同治疗方法分为观察组和对照组,各24例。对照组行常规西医治疗,观察组基于对照组给予破格救心汤加减治疗。治疗10 d后,比较两组患者XDRAB或PDRAB清除率、临床疗效、炎症指标、中医证候积分、急性生理学和慢性健康状况评价系统Ⅱ(APACHEⅡ)评分和氧合指数(OI)。结果治疗10 d后,观察组XDRAB或PDRAB清除率和愈显率高于对照组;两组患者白细胞计数、中性粒细胞、C反应蛋白水平、降钙素原水平、中医证候积分和APACHEⅡ评分均低于治疗前,且观察组较对照组更低;两组患者OI高于治疗前,且观察组较对照组更高(P<0.05)。结论破格救心汤加减治疗伴XDRAB或PDRAB感染重症肺炎阳脱证患者,不仅能够缓解临床症状,降低炎症指标水平,还能提高XDRAB或PDRAB清除率,临床效果较好。Objective To observe the clinical effect of modified Poge Jiuxin Tang in the treatment of severe pneumonia patients with Yang prostration syndrome associated by extensively drug-resistant Acinetobacter baumannii(XDRAB)or pandrugresistant Acinetobacter baumannii(PDRAB)infection.Methods A total of 48 severe pneumonia patients with Yang prostration syndrome associated by XDRAB or PDRAB infection admitted from October 2020 to May 2023 in Zunyi Hospital of Traditional Chinese Medicine were selected as the research objects.According to different treatment methods,they were divided into observation group(n=24)and control group(n=24).The control group was treated with conventional western medicine,while the observation group was added with modified Poge Jiuxin Tang on the basis of treatment for the control group.After 10 days of treatment,the clearance rate of XDRAB or PDRAB,clinical efficacy,inflammatory indexes,traditional Chinese medicine syndrome scores,Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)score and oxygenation index(OI)were compared between the two groups.Results After 10 days of treatment,the clearance rate and the more explicit rate of XDRAB or PDRAB in the observation group were significantly higher than those in the control group.After treatment,the white blood cell count,neutrophil,C reactive protein level,procalcitonin level,traditional Chinese medicine syndrome scores and APACHE II scores were significantly lower than those before treatment in the two groups,and the values in the observation group were significantly lower than those in the control group.After treatment,the OI levels were significantly higher than those before treatment in the two groups,and the level in the observation group was significantly higher than that in the control group(P<0.05).Conclusions Modified Poge Jiuxin Tang in the treatment of severe pneumonia patients with Yang prostration syndrome associated by XDRAB or PDRAB can not only alleviate clinical symptoms,reduce levels of inflammatory indexes,but also impr
