AngioJet机械血栓清除术治疗急性下肢深静脉血栓的临床效果分析

摘　　要：目的分析AngioJet机械血栓清除术治疗急性下肢深静脉血栓(LEDVT)的临床效果。方法选取曲靖市第二人民医院2022年1月—2023年10月收治的60例急性LEDVT患者,按照不同治疗方式分为AngioJet组和导管接触性溶栓(CDT)组,各30例。AngioJet组行AngioJet机械血栓清除术,CDT组行CDT。比较两组患者血栓清除情况、溶栓时间、住院时间、使用尿激酶总量、术后并发症及术前、术后3 d患肢周径差、凝血功能。结果AngioJet组患者血栓总清除率高于CDT组,溶栓时间和住院时间短于CDT组,使用尿激酶总量少于CDT组;术后3 d,AngioJet组患者患肢大腿周径差和患肢小腿周径差小于CDT组,凝血功能优于CDT组,差异有统计学意义(P<0.05)。两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论AngioJet机械血栓清除术治疗急性LEDVT患者可取得较好临床效果,能减轻患肢水肿症状,改善凝血功能,且具有较高的安全性。Objective To analyze the clinical effect of AngioJet mechanical thrombectomy in the treatment of acute lower extremity deep venous thrombosis(LEDVT).Methods A total of 60 patients with acute LEDVT admitted to Qujing Second People's Hospital from January 2022 to October 2023 were selected and divided into AngioJet group and catheter directed thrombolysis(CDT)group according to different treatment methods,with 30 cases in each group.The AngioJet mechanical thrombectomy was performed in the AngioJet group,CDT was carried out in the CDT group.The thrombus clearance,thrombolysis time,hospitalization time,total amount of Urokinase used,postoperative complications,limb circumference difference and coagulation function before and 3 days after operation were compared between the two groups.Results The total clearance rate of thrombus in AngioJet group was higher than that in CDT group,the thrombolysis time and hospitalization time were shorter than those in CDT group,and the total amount of Urokinase was less than that in CDT group.At 3 days after operation,the difference of thigh circumference and calf circumference in AngioJet group was smaller than that in CDT group,and the coagulation function was better than that in CDT group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusions AngioJet mechanical thrombectomy in the treatment of patients with acute LEDVT can achieve more clinical results,reduce the edema symptoms of the affected limb,improve coagulation function,and has high safety.
