腹腔镜完全腹膜外疝修补术治疗腹股沟疝的临床效果观察

摘　　要：目的观察腹腔镜完全腹膜外疝修补术(TEP)治疗腹股沟疝的临床效果。方法选取2021年5月—2023年5月济南市章丘区埠村街道社区卫生服务中心收治的56例腹股沟疝患者,按照不同的手术方式分为观察组和对照组,各28例。对照组行腹腔镜经腹腹膜前疝修补术,观察组行腹腔镜TEP。比较两组患者围手术期指标、疼痛程度、应激反应指标、生活质量和并发症发生情况。结果观察组除手术时间较对照组延长外,其他围手术期指标优于对照组;术后1 d,观察组视觉模拟评分法评分、去甲肾上腺素水平和皮质醇水平低于对照组;术后1个月,观察组生活质量综合评定问卷-74各维度评分高于对照组(P<0.05)。两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论腹腔镜TEP治疗腹股沟疝患者虽会延长手术时间,但能更好地缓解疼痛程度,减轻应激反应,提高生活质量,且并发症少。Objective To observe the clinical effect of laparoscopic totally extraperitoneal prosthesis(TEP)in the treatment of inguinal hernia.Methods From May 2021 to May 2023,a total of 56 patients with inguinal hernia admitted to Jinan Zhangqiu District Bucun Street Community Health Service Center were selected and divided into observation group and control group according to different surgical methods,with 28 cases in each group.The control group underwent laparoscopic transabdominal preperitoneal prosthesis,and the observation group underwent laparoscopic TEP.Perioperative indicators,pain degree,stress response indicators,quality of life and complications were compared between the two groups.Results In addition to the operation time of the observation group was longer than that of the control group,other perioperative indicators were better than the control group.1 d after operation,the Visual Analogue Scale score,norepinephrine level and cortisol level in the observation group were lower than those in the control group;1 month after operation,the scores of each dimension of the Generic Quality of Life Inventory-74 in the observation group were higher than those in the control group(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusions Although laparoscopic TEP treatment of inguinal hernia patients will increase the operation time,it can better relieve pain,reduce stress response,improve quality of life,and have fewer complications.
